Provides an array of community support groups for individuals and families including cancer, amputee, colorectal cancer, Alzheimer, bariatric surgery

A monthly support group for all pre and post amputation patients and their loved ones. Free monthly support groups will offer emotional support, information and fun for all who attend. Group meets the second Saturday of each month.

Service Coordination is used to assist people in the following areas: - Helping people access a full range of quality services that assure the best outcomes, such as independence, health and quality of life. - Finding funding for services - Finding transportation to appointments - Addresses basic physical needs and supports necessary for a stable, safe and healthy community life - Works with consumers to make sure that their voice is heard

Works to maximize the quality of life for veterans and all people with spinal cord injury/dysfunction by advocating for health care, spinal cord injury/dysfunction research and education, veterans' benefits and rights, sports programs, disability rights and accessibility and removal of architectural barriers.

Provides a connection to community resources and services for individuals with physical disabilities. Services include: personal assistance, home adaptations, specialized therapies, assistive technology, skilled nursing, adult daily living services, durable medical equipment, employment services, home delivered meals, non-medical transportation, nursing home transition, personal emergency response, residential habilitation, and vehicle modifications.

Provides education and outreach presentations to businesses, provider agencies, community partners and people with disabilities. Presentations include Accessibility Surveys, Disability Sensitivity and Awareness, and guidance to assure compliance with the ADA.

Helps coordinate services including personal care assistance, home modifications, transportation, and discharge planning. Also administers the OBRA and Independence, and CommCare Waiver Programs.

Provides coordinated resources, case management and social work services to adults who are physically disabled in Chester County and parts of Bucks, Delaware, Lancaster, and Montgomery County. Other services provided include information and referral, home modifications , adaptive equipment, transportation services, Community Service Programs for Persons with Physical Disabilities, personal emergency service systems for people with disabilities who qualify, and referrals to other services.

Provides coordination of services to people with physical disabilities and seniors through Community HealthChoices. Assists in developing plans that may include the following services that enable individuals to live safely and independently at home: personal care attendants, durable medical equipment, home delivered meals, home modifications and supportive employment.

Offers low-income rental housing with accessible units and units to meet the needs of persons with mobility, hearing and vision impairments. Wait list priority for families with a household member who is disabled.

Services include: * Support groups -Caregiver Groups, for women and men caring for spouse/child affected with a disability -Parents with children with disabilities -Singles with disabilities -Young Adults living with disabilities * Speakers available * Wheels for the World, collects and refurbishes old wheelchairs and distributes them worldwide * Special Delivery, gift packages of institutionalized materials delivered to people with disabilities who are hospitalized, homebound or institutionalized * Holds 3 one-week retreats per year for families with a disabled family member * Holds 1 one-week Warrior Getaway retreat for service men and women (with visible and invisible wounds of war) and their families. * Works with local churches to establish disability ministry programs in their congregations

Provides personal care and light household cleaning for non-Medical Assistance individuals with physical disabilities.

Provides non-medical home care services including personal care, companionship, respite care, light housekeeping, and meal preparation for seniors and individuals with disabilities. Services are designed to help clients remain safe and independent in their homes. The Care Together program offers discounted care for additional household members receiving support.

Support for individuals who have been affected by amputation.

Provides opportunities for individuals to participate in a wide variety of social and recreational activities. Emphasis is on community-based activities. Transportation is available for most activities. Includes Summer and special vacaion programs.

Offers multi-event sports and rehabilitation program. Veterans compete in a variety of adaptive sports, including wheelchair basketball, quad rugby, archery, triathlon, track and field, swimming, softball, slalom, nine ball and boccia.
