A monthly support group for all pre and post amputation patients and their loved ones. Free monthly support groups will offer emotional support, information and fun for all who attend. Group meets the second Saturday of each month.

Provides non-medical home care services including personal care, companionship, respite care, light housekeeping, and meal preparation for seniors and individuals with disabilities. Services are designed to help clients remain safe and independent in their homes. The Care Together program offers discounted care for additional household members receiving support.

Provides coordination of services to people with physical disabilities and seniors through Community HealthChoices. Assists in developing plans that may include the following services that enable individuals to live safely and independently at home: personal care attendants, durable medical equipment, home delivered meals, home modifications and supportive employment.

Other services provided include information and referral, home modifications , adaptive equipment, transportation services, Community Service Programs for Persons with Physical Disabilities, personal emergency service systems for people with disabilities who qualify, and referrals to other services.
