Pennsylvania Department of Health - Nurse Family Partnership

A registered nurse visits in the home throughout pregnancy and until the baby is two years old. They assist new parents to build strong networks of support, get referrals for health care, childcare, job training and other support services available in the community, find ways to continue education and set goals for the future.

Geisinger Free2BMom - Free2BMom

Provides a recovery pathway that provides intensive support and behavioral health case management for pregnant individuals, or new moms, who have a diagnosis of substance use disorder (SUD). The goal of Free2BMom is to improve outcomes for mom and baby by reducing stigma and helping new moms or pregnant individuals face all the normal challenges of being a new parent. As well as overcome other obstacles of navigating care, including access to physical and behavioral healthcare, treatment for SUD, transportation, and connection to community resources. Enrolled individuals will receive case management for a period up to 2 years by a master’s level social worker (BHCM) and a certified recovery specialist (CRS).
