Operates an outpatient primary medical care clinic for Veterans through the Lebanon VA. Telehealth: As a convenience to our patients, some services are offered by electronic means. Individual or group appointments may be performed via video to the Lebanon VAMC or other medical facility. Callers will be connected to trained hotline staff who will conduct a brief screening to assess needs; homeless veterans will be connected with the Homeless Point of Contact at the nearest VA facility; family members and non-VA providers calling on behalf of a homeless Veteran will be provided with information regarding the homeless programs and services available; contact information will be requested so staff may follow-up.
