Helps older Pennsylvanians to live independently while receiving services and supports that meet the health and personal needs of the individual. Services may include: LIFE Center primary care, dentistry, emergency services, hospital care, laboratory/x-ray services, medical specialty services, nursing home care, nutritional counseling, occupational and physical therapies, prescription drugs, home care, meals, caregiver training, support groups, approved transportation, and respite care.

Supports individuals with disabilities in finding competitive employment in the community to enjoy a productive, inclusive, and fulfilling life, which may lead to self-sufficiency. Provides services such as individual assessment, career counseling, job referral, job development, job coaching, and ongoing support. Facilitates student transitions to the workforce and offers links to additional support and resources. Supplies assistive technology and coordinates transportation.

Offers an early intervention program for youth who exhibit high-risk or at-risk behaviors. Designed to work with youth and families to prevent truancy, steer youth away from delinquency, improve school performance, enhance life and social skills, and strengthen family relationships. Services include individual and family counseling, home and school visits, after-school group programming three days a week, and case management.

Operates inpatient residential facility for detox and stabilization services. Services for patients addicted to both substances and/or alcohol and/or have co-occurring mental health conditions. Average length of stay is 7-10 days. Referrals are then provided for ongoing treatment at the next level of care ranging from 28 days of residential, PHP, IOP, sober living, or individual counseling.

Offers a a community-based, clinical day program for youth and young adults age 12 to 18. Provides a school credit recovery program, as well as vocational training. Programs are offered during the school-year as well as in the summer.

Redirects first-time, low-level juvenile offenders away from formal court processing. Terms and conditions include probation supervision, payment of supervision fee, restoration of the victim, and any other services deemed necessary to address risk or needs of the youth. Participating youth are supervised by the Community Specialist. After successful completion of the diversion program, the alleged charges are dismissed. Youth Aid Panels provide a restorative response to juvenile crime by involving the victim, offender and community. The Youth Aid Panel provides an alternative to juvenile court. The panel is a group of six trained volunteers who enter into a contract with first-time juvenile offenders. The contract allows the individual to make amends for a crime in exchange for wiping the offense off his or her permanent record. The offense must be a non-violent one. Juveniles must make an unconditional admission of guilt and perform an act of retribution such as community service, participating in rehabilitation and awareness seminar or writing an essay. Failure to fulfill the contract will result in the case being sent to juvenile court.

Together, St. Mary and PRS is focused on meeting patient and community needs, including coordination of behavioral services (not medical) for anyone who is in need of drug relapse prevention. This includes department of corrections, recovery houses, behavioral health centers, physician practices, and individual patients who self-refer. Through PRS, patients have access to Medication-assisted treatment therapy, case management, therapeutic programs, drug and alcohol counseling.

This is accomplished with an array of services for the individual, including career counseling, vocational profile development, situational assessment, job development and placement and job coaching. Job coaching follows the consumer to their new workplace environment, giving the support and assistance while they learn job duties and responsibilities, including specific job tasks and work assignments.

Offers aid to families who find themselves in a kinship care situation. Provides support services which include information to caregivers about available services; assistance to caregivers in gaining access to the services; individual counseling, organization of support groups, training of caregivers to assist them in decision-making and problem-solving that are related to their caregiving roles; and reimbursement for services and/or supplies.

Provides a supportive service that is designed to assist children in functioning more effectively and handling problems facing them. It is an in-home program consisting primarily of activities, tutoring and individual counseling provided by a professional advisor. Those who are eligible for the Advisor Program are individuals referred from Probation, Children and Youth clients who are referred by agency caseworkers and /or C.A.S.S.P. clients.

Provides confidential assistance to any victim of domestic violence, sexual assault, or other crime, recent or past. Services include emergency domestic violence shelter, crisis services, transitional and supportive housing, medical and legal advocacy, civil legal representation, counseling for adults and children, and prevention education.
