Provides eligible individuals permanent housing assistance through LCCAP.

Partners with the Office of Behavioral Health and the Delaware County Housing Authority to provide housing vouchers to persons experiencing homelessness. Through various housing processes with the Continuum of Care and Coordinated Entry, homeless singles and families are identified and referred to FCSDC to identify safe and affordable housing.

Provides supportive housing for single homeless participants who suffer from mental health and/or substance abuse. Selected candidates are provided ongoing financial assistance in the form of rental subsidy vouchers that can be used to secure a housing with an account incentive. Through intensive case management clients are assisted with budgeting, household management, job training, and employment support.

Offers 14 two-bedroom and 7 three-bedroom townhomes for rent. Two units are single story and have accessible features.

Provides housing and case management for families experiencing homelessness. Families can stay in the program for up to 12 months while working on long and short term goals with a BCHG case manager.

Provides case management, transitional housing and supportive services to homeless women and their children. Supportive services include case management, counseling, and schooling support for each child.

Supplies short-term rental subsidies and supportive services to families. Assists participants with services such as housing search, case management, budgeting, and job training, as well as goal planning and guidance toward self-sufficiency for up to two years. Assists clients with appropriate home furnishings to enable them to move forward on the road to independent living.

Offers 24 garden style apartments and 16 townhomes for rent. There is one accessible unit in each building.

Provides connections permanent housing through a use of time-limited financial assistance and supportive services. Can assist with first month's rent, deposit, short rental subsidy (1-3 months), and medium rental subsidy (3-6 months).

Encourages and assists families to become self-sufficient. Participants are assigned a coordinator to help them set and reach goals related to becoming self-sufficient, such as receiving more education, job training, help with childcare, a career, home ownership, and any other goals they may have. Offers a financial incentive to families through an escrow account (see Helpful Tips for more information).

Acts as the Coordinated Entry Agency for housing needs in Butler County. Potential services include emergency grants, permanent supportive housing for individuals and families with children, in particular those struggling with mental illness, and temporary/transitional housing and support

Offers semi-supervised housing programs for young adults to transition from homelessness. Provides services to individuals and families that may otherwise not qualify for housing with property management companies or other landlords, due to poor credit, limited income, or inability to pass a background check.

Provides services including case management, employment readiness and placement, and mental health and substance abuse treatment. Provides 11 leased apartments and supportive services for individuals and families experiencing homelessness in Allegheny County. Offers 24/7 staff support.
