Long-term care assessments are also completed for those applying for a personal care home Social Security supplement, and for other DPW programs, such as Waiver Services. Facilities and individuals may also request a level of care determination. Case management help older adults to assess their abilities and needs, explore options, develop plans to best utilize their abilities, and arrange for informal and formal supports (in-home services) to meet their needs. With an older adult looking for placement, they can assist older adults who need to enter a nursing home or personal care facility with determining the type of residential placement that will best meet their needs, and with securing such placement. They can also assist those in nursing facilities.

Care managers have knowledge of State and Federal programs that provide needed in home support. Using comprehensive assessments and person centered care planning, care managers work with older adults to create a personalized plan for the future.

Services include geriatric assessments, medication reviews, memory evaluations and preventive care. Provides wellness classes and activities for older adults.

Specializes in providing mobile assessments for older individuals who are experiencing a change in mental status, behavior or mood. Can be completed in a familiar setting at the convenience of the individual and loved ones.

Geriatric outpatient assessment center; that provides consultations, evaluations, and recommendations based on the individuals medical history, health condition, current medicines and needs.

Provides primary care medical services ranging from pediatric to geriatric care. Accepts most third party payments. Patients without insurance asked to pay whatever they can. Home visits to homebound elderly available. Also offers personal counseling and smoking cessation programs.

Senior Assessment Program at the Geriatric Center provides comprehensive evaluations performed by a geriatrician, a psychiatrist, and a licensed clinical social worker to lay the foundation for quality ongoing care. This care assists an older individual with the ability to enjoy good health, improve overall quality of life, reduce the need for hospitalization, plan for long-term care, as well as live independently for as long as possible.
