Program, generally staffed by an interdisciplinary team comprising a geriatrician, a nurse, a social worker and a pharmacist, that evaluates the functional ability, physical health, cognitive and mental health and socioenvironmental situation of older adults, particular those who are frail or chronically ill, to identify health-related problems, develop plans for treatment and follow-up, coordinate care, determine the need for long-term care, and ensure the optimal use of health care resources. Beneficial outcomes may include greater diagnostic accuracy, improved functional and mental status, reduced mortality, decreased use of nursing facilities and acute care hospitals and increased satisfaction with care.

Offers a wide range of support services to older Lancaster County residents with the goal of allowing them to maintain their independence and self-sufficiency. Most services available to persons ages 60 and over, except for the 55+ Job Bank, Job Bank Workshop and Senior Community Services Employment Program which begins at age 55 and older. Participants will meet with a physician and a social service person for an evaluation of their physical, emotional and cognitive health. The senior will be given a list of recommendations that the team has made to share with their personal physician. Geriatric Assessment
Complete assessments to determine what level of care (i.e. nursing home, personal care home or community services) is appropriate for individuals on Medical Assistance to pay for a nursing home stay. A physician's medical report is needed to start the process. Long-term care assessments are also completed for those applying for a personal care home Social Security supplement, and for other DPW programs, such as Waiver Services. Facilities and individuals may also request a level of care determination. Case management help older adults to assess their abilities and needs, explore options, develop plans to best utilize their abilities, and arrange for informal and formal supports (in-home services) to meet their needs. With an older adult looking for placement, they can assist older adults who need to enter a nursing home or personal care facility with determining the type of residential placement that will best meet their needs, and with securing such placement. They can also assist those in nursing facilities.
