Purpose of services is to reduce unplanned pregnancies by supporting women's choice to space their children or to remain childless, and to provide comprehensive gynecologic medical services. Services include physical exams; tests for STDs (including HIV); counseling about safe sex, abstinence, and contraceptives; contraceptive methods at no cost to patients; and pregnancy testing.

Provides pregnancy tests, options education, STD education and testing, parenting support and materials assistance.

Provides medical case management services to persons living with HIV/AIDS and persons at-risk for the disease, including food and nutrition assistance, transportation assistance, psychosocial and mental health resources, dental assistance, and housing assistance (HOPWA). Also provides HIV and STD testing and programming for prevention, education, and outreach throughout Montgomery County.

Services include communicable disease reporting and investigation; epidemiology, informational and referral; chronic disease prevention and intervention; clinical services (including sexually-transmitted disease and tuberculosis diagnosis and treatment, immunization, and HIV testing, counseling and education); family health programs and environmental health services (including animal bites and rabies information).

Provides gynecological and reproductive health care services; as part of the state-funded Healthy Beginnings program provides eligible pregnant women with prenatal care, psychosocial services, nutrition education, and other support services.

Offers emergency financial assistance as a last resort to individuals and families effected with HIV or AIDS.

Health centers are the implementation sites for the Department of Health's public health programs. Current Services: - Free HIV Rapid Testing (Oral mouth swab with results in 20 minutes) is provided without an appointment Monday through Thursday, 9:00am to 11:00am and 1:00pm-4:00pm, Friday, 9:00am to 11:00am - Education information on labs and medications related to the treatment of HIV/AIDS - Primary Medical Services - Referrals for Mental Health - Referrals for Drug and Alcohol Counseling - Nutritional Counseling - Social Work Case Management - Meets the collective needs for persons with HIV/AIDS. LGHP-CC is a Ryan White Federal grant site. - Four doctors on staff who specialize in HIV/AIDS and are board-certified Family Medicine Physicians.

Provides adult and child immunizations, HIV/AIDS referrals, and health resources.

STD/HIV services include testing and treatment of chlamydia, gonorrhea and syphilis, as well as testing and referral services for HIV. Partner notification of possible exposure to STDs and HIV is available and serves to increase testing and treatment to reduce the spread of disease.

Provides health education, information and referral services targeted at the Latino community. Offers prevention education on drug/alcohol abuse and tobacco use and provides HIV testing, hepatitis testing and screening for chronic conditions such as diabetes. Individuals requiring other medical services are provided with referrals. Benefits check-up services are available to older adults. Community health worker conducts street outreach and home based health education services.

Offers the following services: - Immunizations (all ages), offered by appointment Thursday, 9:00-11:30am & 1:00-2:30 pm. Immunization record must be brought along. Immunizations for measles, rubella, and mumps can be given between ages 12-15 months, others usually 2 months. The following immunizations are offered: DTP (diphtheria, tetanus, pertussis) IPV (Inactivated Polio Vaccine) Prevnar (Pneumocial vaccine for infants) MMR (measles, mumps, rubella) Hepatitis B-Hepatitis A Gardasil (Vaccine used to prevent cervical cancer and precancerous cervical lesions) HIB (hemophilus influenza) DT Booster or TD (for older children) Varicella (chickenpox) TDYS - Tuberculosis Clinics provide confidential diagnosis, treatment (including medication), and follow-up. (Will do a free testing for persons who have been exposed to the germ). TB clinic by appointment - HIV/AIDS antibody testing and pre- and post-test counseling. Free, confidential, and anonymous.

Provides clinic with confidential and anonymous testing and counseling for HIV and other Sexually Transmitted Infections (STIs), pregnancy, and Hepatitis C. Also provides primary care, family planning services, general health screenings, and at-home HIV testing kits.

Provides case management, education, harm reduction, and support programs for straight and LGBTQ+ youth of color. Also offers support groups, HIV and STI resources, safer sex and hygiene kits, PrEP referrals, mental health and substance use services, food and clothing services, and work and job readiness training.

Provides primary health care and dental services, including routine well-care with disease prevention screening, immunizations, women's health, non-emergent acute care, and continuity care management of chronic medical conditions.
