Medication reviews with a registered pharmacist from Evangelical Community Hospital are offered annually at each senior center and free blood pressure screenings are offered by licensed nurses at each senior center.

Provides health wellness programs for condition management through the use of comprehensive, goal-oriented wellness planning.

Prevents youth violence, delinquency and drug use, and promotes positive youth development and strong families.

Provides nutrition counseling and education from Registered Dietitians (RDs) and Licensed Dietitian-Nutritionists (LDNs), who draw on their experience to develop individualized plans that translate nutritional science into practical guidance for healthy living for individuals of all ages. Supports healthy weight management, improved eating habits, and reduced risk of chronic illness. Creates a specialized nutrition plan to help individuals better manage conditions such as diabetes and pre-diabetes, heart disease, high blood pressure, kidney disease, cancer, celiac disease, eating disorders, food allergies, pregnancy (including gestational diabetes), child and adolescent nutrition, and vegetarian diets.

Provides one hot, and one cold meal, Monday through Friday, to home bound seniors.
