Offers assessments by licensed optometrists, prescriptions for optical aids, and the opportunity to try out aids ranging from magnifiers, writing guides, special lighting devices, and closed-circuit TVs. Prevention of blindness programs including free vision screenings for children 12 months to 6 years of age. Transportation: This free door-to-door service provides rides to essential medical appointment or critical service need, such as obtaining a photo ID or grocery shopping. Individuals must live in Allegheny County and meet the program criteria. Appointments requested to be made in advance.

Provides non-medical personal assistance home care, home health nursing care and skilled physical, occupational and speech therapist. Services are provided in the individuals home or other independent living environments. Accommodates ongoing care, one-time-only requests when regular caregiver/family member is out of town.

Provides short-term rehabilitation and long-term skilled nursing, post acute care following a hospital discharge, respite care, specialized Alzheimer's and dementia care and hospice services. A rehabilitation department with licensed physical, occupational and speech therapists.

Offers physical therapy, occupational therapy, speech therapy, developmental teaching by special instructors, and infant massage. PennCares preschools allow young children with and without disabilities to play, develop, and learn together while promoting a sense of belonging.

Provides in-home services by registered nurses, LPN's, home health aides, physical therapists, occupational therapists, and social workers. Does not offer private-duty nursing.

Provides a range of health care services including: speech/occupational/physical therapy, IV therapy, central line management, whirlpool therapy, wound care management, short-term and long-term stays, respite care, 24 hour nursing care, certified hospice program, licensed social worker, specialized dietary services, parenteral or enteral nutrition, organized stimulation activities at all ability levels, private or semi private rooms, secure care system for high level of safety, visiting hours 24 hours/day, 7 days/week.

Help with increasing muscle strength, improving mobility, restoring function, or adapting to a permanent disability. Offers a person-centered approach to rehabilitation and takes time to understand needs to create meaningful treatment plans.

Provides physical, occupational, and speech therapies after hospitalization. Provides medically-necessary discharge planning for return to home, including services in home.

A rehabilitation program that develops an individually tailored treatment plan that combines the resources of physical, occupational, and speech/language therapists; physiatrists (physical medicine specialists); neuropsychologists/psychiatrists; cognitive rehabilitation therapists; rehabilitation nurses; vocational counselors; social workers, and other specialists to help individuals who have been disabled by an acquired brain injury attain their maximum level of functioning and quality of life.

Provides a variety of individualized outpatient programs, which may include physical therapy for patients with joint replacements, neck, back and shoulder pain, sports injuries, work-related injuries, orthopedic injuries, neurological disorders, pain management, hip and knee rehabilitation, arthritis rehabilitation, occupational therapy, hand rehabilitation, wound care, speech and language therapy, burn care, vestibular rehabilitation, dysphagia programs, lymphedema therapy and discharge planning.

Offers a full cycle rehabilitation program, including physical therapy, occupational therapy, and speech pathology.

Provides 24-hour support and skilled nursing services to those dealing with the challenges associated with advanced age, a prolonged illness or a chronic condition.

Our short-term rehabilitation services are designed for individuals needing an extensive rehabilitation program following an illness, surgery or injury. We strive to restore original ability levels and prepare patients for independent living in their homes.
