Provides In-home services for long term health care and transitional mental health care.

Offers non-medical care at home for basic housekeeping tasks and errands. Also offers physical, occupational, speech pathology and respiratory therapies, both short term and long term basis. Rehabilitation services are available 7 days a week.

Provides medical, health and social services that promote independence at home, for as long as desirable and feasible. Serves seniors with a range of disabilities including chronic physical illness, memory loss, mental illness or mental retardation that impacts activities of daily life.

Offers nursing services, physical, occupational, and speech therapy, home health aide services, and medical social work services to help older adults to maintain independence at home.

Provides elderly or chronically ill veterans with coordinated care that focuses on individual needs, enabling each patient to enjoy the highest quality of life possible. Provides assessment, prevention, stabilization and rehabilitation to eligible residents. Services that are provided fall into the following categories: Hospice and Palliative Care, Home Health Care and Adult Day Health Care, Community Living Center Care and our outpatient Golden Memory Clinic.

Pre-admission involves a participant applying for placement into a nursing facility with Medicaid paying for stay. Also assists with applying for a Personal Care Home medical assistance supplement in Centre County.

Provides the following services for in-home care; skilled nursing, physical therapy, occupational therapy, speech therapy, social work and home health aide.

Provides in-home dialysis services.
