The SBBH Team Program is a mental health program designed for youth ages 5 to 13 years who are experiencing social, emotional, and or behavioral difficulties that impair their ability to function in school and other settings. Provides a "clinical home" for youth and families, ensuring that services are accessible, coordinated across child serving systems, integrated with school interventions, and comprehensive in nature. The SBBH Team is based in the school but delivers services to the child and family in the school, home and community settings. By acting as the single point of contact for children and families, the team will assume clinical responsibility for treatment interventions, coordination of care and case management activities.

Develops individualized plans to help decrease negative target behaviors and replace them with more appropriate behaviors. Provides one-on-one support to children and their caregivers based upon interventions developed by the clinical team.

Offers mental health consultations to early learning programs to: A) Observe a young child in the classroom B) Identify strengths/needs and develop an individual plan that addresses the child’s social-emotional development C) Communicate concerns to parents D) Provide a family with referral information for a child who needs specialized services.

Provides family and child-centered services in the community to support a child and their family to reduce and /or replace problematic behaviors with socially acceptable behaviors.

Specialized service coordination for children of all ages who have a mental health diagnosis and live in Allegheny County.

Connects families with the supports that best fit their specialized needs. Our specializations include the following areas: Introduction to Adult Services, Referrals to experts in areas of financial planning, Assistance with Supplemental Security Income (SSI) applications, Education consultation, Referrals to support groups and on-line resources, Assistance with applications to: Medical Assistance, Office of Vocational Rehabilitation and Waiver programs, General coordination of current services received from other providers, and Person-centered planning.

Provides Behavioral Specialist Consultant, Therapeutic Staff Support, and Mobile Therapy to youth with an Axis I diagnosis. YAP's rehabilitative process is focused on guiding families to realize practical, achievable options for optimum family living through a solid understanding of the diagnosis, as well as resiliency through empowerment. The service currently works with roughly 120 youth and families.
