Program that offers the services of paraprofessional aides who provide assistance with personal hygiene (bathing, grooming and mouth care), clothing care, ambulation, seating, toileting, housekeeping (changing bed linens or other chores that are essential to the individual's health and comfort), food preparation and nutritional and environmental support for recently discharged hospital patients, elderly people and people with disabilities in their own homes or other settings. Personal care may also include supervision which involves cueing, reminding, prompting or directing daily activities, as needed, but does not include medical services.

Each individual is evaluated to determine which living environment will work best for them depending on their needs, wants, and goals. Behavioral Services - Behavior support services use empirically-validated practices to identify the causes of, intervene to prevent, and create appropriate reactions to problematic behavior. Vocational Training - Vocational Training provides the supports individuals need to successfully find and maintain a job. Individuals receive assistance to build their resume and discover their skills and abilities. Companionship Services - A companion is a Direct Services Professional that provides support in the individuals own home while

Assessment of need is done through our office and we work with volunteers and subcontractors to deliver appropriate service. Some of the services are: A) Home Delivered Meals: A meal is provided to functionally disabled, home-bound, or convalescent people age 60+ B) Personal Care Service: Provides personal grooming help - bathing, dressing, feeding, toileting and transferring C) Respite Care: Program designed to provide relief to the primary caregiver. RESPITE CARE: sole caregiver of a consumer is able to leave the home for short periods weekly for business or recreational purposes.

HOME SUPPORT SERVICES: assistance with laundry and grocery shopping for up to 2 hours every other week. Note: laundry and grocery shopping are private pay.

Trained care assistants from contracted Home Health Agencies provide the services authorized by the care manager.
