Provides a street outreach program designed to help runaway, homeless, nomadic and street youth in Bucks County, Philadelphia, Allentown, and Dauphin County. The program offers survival supplies (food, clothes, hygienic products, sleeping bags, tents, tarps, etc.), peer support, informal counseling (individual, family, or group), information and referral services and assistance to get off the streets, if desired.

Assists with shelter search and connections to housing providers for people who are homeless or at-risk of homelessness.

Meets people where they are living outdoors to provide basic items needed for survival, such as tents, tarps, sleeping bags, water, and socks.

Provides permanent housing and support services, coordinated entry, and street outreach for people who are or were experiencing homeless or are at risk of homelessness. This does not serve as emergency housing.

Operates 5 days a week. Outreach workers go out to neighborhoods, parks, and other places where youth who are facing homelessness seek refuge. Offers food, drink, blankets, hats, gloves and scarves, counseling, and referrals to youth who are on the street or unable to come into shelter.

Provides mobile street outreach solutions to address the challenges of violence, being unhoused, and overall community disconnection. Offers support and gear such as sleeping bags, items to help in extreme temperatures, hygiene items, and other items as available. Connects callers and visitors to local resources.

A day center that provides services case management, linkage to mental health services, linkage to drug and alcohol services, medical clinic, dental clinic, and vocational support. . Other basic services like bathroom and showering facilities, as well as laundry facilities, are also available. Street 2 Feet is a program under RHD Crossroads Community Services. There are no overnight services available with this program.

Provides a 30 day emergency shelter for women, men and children, providing a safe place to stay while working to secure permanent housing, case management, life skills education, employment readiness, and financial literacy.

Services include homelessness prevention, help to regain housing stability; Street Outreach to connect unsheltered homeless individuals or families with emergency shelter, critical health and mental health services; and Rapid Rehousing for quick return to permanent housing. May also help with your security deposit and/or rent.

Provides a continuum of care for chronically mentally ill and substance abusing homeless populations. Includes shelter outreach, day therapeutic programming for the dually diagnosed, outpatient, and supported independent living. Programming is accessible through the homeless shelter system or through the Alternative Services Unit of the Behavioral Health.
