Makes connections to street outreach for individuals experiencing literal homelessness in Allegheny County. Assesses eligibility for housing programs for people experiencing literal homelessness in Allegheny County.

Street Outreach

Engages, assesses, and combats homelessness for individuals living on the streets, in encampments, and other locations not intended for human habitation. Outreach staff work to build trust with those experiencing homelessness first by helping to meet some of their basic needs (including providing a mailing address to receive mail, food, clothing, showers, laundry, transportation, identification, etc.). The street reach coordinator collaborates with everyone to access housing, employment, medical care, mental health care, substance abuse treatment, and other services while advocating for housing solutions for clients. Offers showers and laundry any day of the week from 9:00am to 12:00pm and 3:00pm to 5:00pm. There is a sign-up sheet out front and then admitted into the building into the lobby to wait for the shower time. Coffee and snacks, free clothing bins, winter coats, tents, sleeping bags, and mail pick-up are also being served while waiting.

Operates 5 days a week. Outreach workers go out to neighborhoods, parks, and other places where youth who are facing homelessness seek refuge. Offers food, drink, blankets, hats, gloves and scarves, counseling, and referrals to youth who are on the street or unable to come into shelter.

Distributes supplies and services to displaced individuals and families of Schuylkill County. Provided are emergency food, clothing, and hygiene supplies. Supplies are made available by donation and are limited. Street Outreach Workers reach out to the unsheltered and connects them with emergency shelters, housing or critical services. Also provides urgent, non-facility-based care to unsheltered who are unwilling or unable to access emergency shelter, housing or appropriate health facility.

Emergency Services and Crisis Intervention services provide JFK Mobile Emergency Team, Central Intake / Crisis Intervention, Crisis Specialist and Outreach to the Homeless. Crisis intervention at any location is available 24 hours a day, 7 days a week. An outgrowth of the JFK Mobile Emergency Team is the Crisis Specialist Program. Crisis Specialist professionals meet the consumer at his or her location, stay to stabilize the situation and determine if further care is needed. This service is also available through the same delegate line as the JFK Mobile Emergency Team. Central Intake / Crisis Intervention provides a central access point to help reduce the use of psychiatric hospitalization and connect people to services with the least disruption possible to their everyday lives. Outreach to the homeless is ongoing and takes place in various areas of the city as the need arises.

Provides a continuum of care for chronically mentally ill and substance abusing homeless populations. Includes shelter outreach, day therapeutic programming for the dually diagnosed, outpatient, and supported independent living. Programming is accessible through the homeless shelter system or through the Alternative Services Unit of the Behavioral Health.

Assists with shelter search and connections to housing providers for people who are homeless or at-risk of homelessness.

Assist street homeless men and women to connect to community resources as they strive to move from homelessness to self-sufficiency. * Social Security Representative Payee Program: Provides financial monitoring and fund administration to individuals receiving social security who are unable to manage their own finances.

An emergency street outreach program providing case management and social service navigation assistance to individuals experiencing homelessness. Meets individuals where they are and helps to address their most basic needs. Conducting client-led case management allows clients to address concerns they may have and reach the assistance they may need/want.

A day center that provides services case management, linkage to mental health services, linkage to drug and alcohol services, medical clinic, dental clinic, and vocational support. . Other basic services like bathroom and showering facilities, as well as laundry facilities, are also available. Street 2 Feet is a program under RHD Crossroads Community Services. There are no overnight services available with this program.
