Also provides urgent, non-facility-based care to unsheltered who are unwilling or unable to access emergency shelter, housing or appropriate health facility.

Provides case management for families (parent[s] with children) dealing with homelessness. Families live at host congregations for one week at a time and rotate around to different churches in the area that provide shelter, three free meals a day, and often provide evening activities and fellowship for the guests. Each day, families are transported to the Day Center by the FPHCR bus where families can do laundry, shower, and get ready for school or work. Parents who are not working stay at the Day Center and work on job and housing searches. Each family meets weekly with a staff member at FPHCR for case management.

Provides a 30 day emergency shelter for women, men and children, providing a safe place to stay while working to secure permanent housing, case management, life skills education, employment readiness, and financial literacy.

G-PATH Housing Outreach Specialist will initiate face to face contact and establish a working interaction with a homeless or at risk of being homeless individual or family to assist them with obtaining adequate shelter and facilitate access to additional services.

Provides 24/7 crisis intervention through the JFK Mobile Emergency Team, Central Intake, Crisis Specialists, and homeless outreach. Services include on-site crisis response, stabilization, assessment for further care, reducing psychiatric hospitalizations, connecting individuals to services, and ongoing outreach in the community.

Street Outreach

Provides temporary emergency shelter. Provides men with food, shelter, spiritual counsel, casework, and networking.

Provides a continuum of care for chronically mentally ill and substance abusing homeless populations. Includes shelter outreach, day therapeutic programming for the dually diagnosed, outpatient, and supported independent living. Programming is accessible through the homeless shelter system or through the Alternative Services Unit of the Behavioral Health.

An emergency street outreach program providing case management and social service navigation assistance to individuals experiencing homelessness. Meets individuals where they are and helps to address their most basic needs. Conducting client-led case management allows clients to address concerns they may have and reach the assistance they may need/want.

Meets people where they are living outdoors to provide basic items needed for survival, such as tents, tarps, sleeping bags, water, and socks.
