Offers services for individuals with substance use disorders. Provides screening and level of care assessment for substance use disorder, referral to treatment, and support until admission if space is not available in an appropriate setting immediately. Care management includes connections to other human services (through warm hand-offs), information and education about treatment options, peer support, and coordinated linkage to treatment and/or services. Also offers a Safe Place to Stay providing food, shelter, and professional support while individuals are waiting for appropriate treatment placement in the community.

Provides mandatory medical assistance for behavioral health services in Allegheny County. Community Care helps users get care for a mental health condition, an alcohol problem, or a drug problem. As a behavioral health managed care organization, Community Care pays for medically necessary mental health and substance use treatment services for HealthChoices/Medical Assistance members, including recovery-oriented services for people with mental illness and substance use disorders. Community Care contracts with a variety of mental health and substance use disorder providers in and around Allegheny County.

Operates an outpatient primary medical care clinic for Veterans through the Lebanon VA. Offers primary care and annual physicals by appointment, preventive services, women's health services, routine lab work, EKGs, immunizations, medication review, tele-mental health and social work services, treatment of conditions such as diabetes, hypertension, respiratory disease, common cold, influenza, tobacco cessation and specialty referrals. Telehealth: As a convenience to our patients, some services are offered by electronic means. Individual or group appointments may be performed via video to the Lebanon VAMC or other medical facility. Also, various tests can now be performed at any one of the six outpatient clinics, with the analysis being performed at the Lebanon VAMC or other medical facility. Inpatient Rehabilitation
Offers individuals without insurance funding assistance available from the Chester County Department of Drug and Alcohol Services. For uninsured individuals assessment for treatment must be done through one of multiple licensed points of entry, including Mirmont Outpatient Services. Assessment is needed to determine what level of care and intensity of service is necessary. Based on the assessment, a referral will be made to the appropriate treatment program.
