ASC is funded by grants from HRSA (Ryan White), and subcontracts for grant funds via AIDSNET. There are two locations for HIV prevention/education, confidential/anonymous HIV testing, comprehensive clinical care (integrated behavioral health; integrated dietitian counseling), and medical case management for HIV-positive patients. We offer PrEP as a preventative for HIV-negative patients. New patient and client intakes are by appointment only. No walk-ins. Must bring documentation of HIV-positive status. HIV/AIDS Prevention/education services are provided for residents of Northampton, Carbon and Monroe County. Location Two clinic locations: 1. Family and Community Service of Delaware County - HIV/AIDS Medical Case Management

The goal of Medical Case Management (MCM) is to empower persons who are living with HIV to maintain optimal physical and emotional health through ongoing assessment of needs, linkage to medical care, retention in medical care, treatment adherence, service planning and monitoring, coordination of services and advocacy.Our Medical Case Managers meet with their clients at least once a month to update goals and to determine progress. Case managers meet with clients in the office, in their homes, or any community setting that is convenient. The following services are available for our MCM consumers: 340B Pharmaceutical Program, Transportation Assistance, Food Vouchers, Client Financial Fund, Support Group, and Peer Mentorship.
