The goal of Medical Case Management (MCM) is to empower persons who are living with HIV to maintain optimal physical and emotional health through ongoing assessment of needs, linkage to medical care, retention in medical care, treatment adherence, service planning and monitoring, coordination of services and advocacy.Our Medical Case Managers meet with their clients at least once a month to update goals and to determine progress. Case managers meet with clients in the office, in their homes, or any community setting that is convenient. The following services are available for our MCM consumers: 340B Pharmaceutical Program, Transportation Assistance, Food Vouchers, Client Financial Fund, Support Group, and Peer Mentorship.

Provides client-centered services that link HIV-positive clients to necessary health coverage and medical treatments while assisting clients in maintaining adherence to treatment.

Provides an independent living facility for people who are HIV positive or living with AIDS.

Offers comprehensive navigation and support to those infected and impacted by HIV/AIDS.

Provides comprehensive, culturally-sensitive medical and behavioral healthcare services for the LGBTQ2+ community.

Offers medical care, mental health, nutrition, and social work for adults living with HIV.

ASC is funded by grants from HRSA (Ryan White), and subcontracts for grant funds via AIDSNET. There are two locations for HIV prevention/education, confidential/anonymous HIV testing, comprehensive clinical care (integrated behavioral health; integrated dietitian counseling), and medical case management for HIV-positive patients. We offer PrEP as a preventative for HIV-negative patients. New patient and client intakes are by appointment only. No walk-ins. Must bring documentation of HIV-positive status. Provides confidential counseling, education, and referrals regarding HIV/AIDS including testing, prevention, and related care.

Provides Medical Case Managers who work in partnership with clients to remove practical and emotional barriers to primary medical care, access to medications, housing, benefits and entitlements, and transportation services.

Provides medical case management services to persons living with HIV/AIDS and persons at-risk for the disease, including food and nutrition assistance, transportation assistance, psychosocial and mental health resources, dental assistance, and housing assistance (HOPWA). Also provides HIV and STD testing and programming for prevention, education, and outreach throughout Montgomery County.

Provides case management, housing opportunities, and permanent supportive housing for people with HIV/AIDS.

Provides supplemental food assistance to eligible low-income families in which one or more members of the household is living with HIV/AIDS.

Offers a range of services to individuals with HIV/AIDS and mental illness and their families or caregivers. Services include counseling for loved ones affected by HIV/AIDS and referrals as needed to other programs within an active network of HIV/AIDS service providers.

Case management involves: A) 4-10 sessions of client-centered, harm reduction-focused one-on-one counseling B) Help getting prevention services C) Assistance coordinating social services, including medical, food, and other benefits D) Connection to legal services E) Connection to medical services Comprehensive Risk Counseling Services sessions focus both on HIV prevention and harm reduction messages and on the immediate needs presented by the client.

Hosts Ryan White Program Peer Support Group, monthly.

We also offer free and confidential HIV testing, walk-in or by appointment, with results in as little as one minute.

Hold biweekly meetings for HIV/AIDS clients, their family members and loved ones. Provides a confidential and supportive environment in which attendees are able to discuss the daily challenges of living with their health condition.
