Provides support to individuals and families who are at risk of experiencing homelessness, housing instability, and/or food insecurity. Provides a day shelter providing an immediate and temporary place for individuals and families experiencing homelessness to stay during the day hours while they seek to regain permanent housing stability.

Housing Case Management Services helps low income families find affordable housing with housing referrals and counseling on budgeting and tenancy readiness. Limited funds available for rental assistance on a case by case basis for their clients.

Operates confidential hotline that provides infomation about programs for veterans experiencing or at risk of homelessness.

Offers daytime drop-in services for Veterans experiencing homelessness and Veterans who are at risk of becoming homeless. Provides daily van transportation to and from designated locations including local shelters, the Corporal Michael J. Crescenz VA Medical Center, and Camden, New Jersey. Services include case management, breakfast and lunch, showers, personal hygiene items, haircuts, and laundry facilities.

Provides case management, transitional housing and supportive services to homeless women and their children. Supportive services include case management, counseling, and schooling support for each child.

Assists participants to develop goals, through counseling, financial planning assistance, personal mentoring, and case management coordination services.

Provides vouchers for shelter for people facing eviction, needing a security deposit to secure housing, or needing overnight shelter. Provides utility vouchers for those in danger of having their gas, electric, or water turned off. Provides food vouchers for transients needing a meal after food bank hours. Provides transportation for those stranded and unable to get transportation out of the community. Other emergency situations or other monetary crises on a case-by-case basis.

Provides assistance accessing benefits, and shelter to Veterans with limited or no access to care. Helps find shelter for Veterans who, due to health or means, are unable to find or keep shelter of their own, and Post Traumatic Stress resources to Veterans through our Veterans Unstoppable program. Provides assistance with nationwide integrated medical care in the future. Provides case management for families (parent[s] with children) dealing with homelessness. Families live at host congregations for one week at a time and rotate around to different churches in the area that provide shelter, three free meals a day, and often provide evening activities and fellowship for the guests. Each day, families are transported to the Day Center by the FPHCR bus where families can do laundry, shower, and get ready for school or work. Parents who are not working stay at the Day Center and work on job and housing searches. Each family meets weekly with a staff member at FPHCR for case management.

Provides personalized case management to help individuals and families in Schuylkill County access essential services and achieve stability. Services include: - Utility Assistance: PPL and UGI utility support. - Emergency Services: Food, clothing vouchers, bus tickets, prescription help, diapers, baby items, and hygiene products. - ID and Documentation: Replacement or renewal of Pennsylvania photo IDs and replacement of birth certificates. - Outreach & Referrals: Connections to community resources and services. - Comprehensive Case Management: Personalized support to overcome barriers and achieve goals.
