Provides personalized case management to help individuals and families in Schuylkill County access essential services and achieve stability. Services include: - Utility Assistance: PPL and UGI utility support. - Emergency Services: Food, clothing vouchers, bus tickets, prescription help, diapers, baby items, and hygiene products. - ID and Documentation: Replacement or renewal of Pennsylvania photo IDs and replacement of birth certificates. - Outreach & Referrals: Connections to community resources and services. - Comprehensive Case Management: Personalized support to overcome barriers and achieve goals.

Participants live in their own housing in the community and receive 3+ months of case management. This may include budgeting guidance, landlord advocacy, education about available resources, and short-term rental assistance. Keeping individuals and families in their own housing is our top priority. Their available services assist over 300 men, women, and children per year, and include the following: --Case management and goal planning --Resource development --Life skills & parenting support --Mentoring/volunteer support --Appropriate levels of financial assistance for rent, utilities, transportation

Assists individuals in securing financial resources, applying for pertinent documents, searching for housing and applying for housing. In some cases, multiple months of rent may be paid along with a security deposit. Other supports, include furniture, household supplies, payments for accessing documents and other unique needs.

Intensive case management for homeless individuals with a serious and persistent mental health diagnosis. Case managers connect with housing options, mental and physical health care providers, and build support systems. Must have Magellan insurance.

Provides homeless shelter and case management for women and children. Donations welcomed.

Provides case management for families (parent[s] with children) dealing with homelessness. Families live at host congregations for one week at a time and rotate around to different churches in the area that provide shelter, three free meals a day, and often provide evening activities and fellowship for the guests. Each day, families are transported to the Day Center by the FPHCR bus where families can do laundry, shower, and get ready for school or work. Parents who are not working stay at the Day Center and work on job and housing searches. Each family meets weekly with a staff member at FPHCR for case management.

Operates confidential hotline that provides infomation about programs for veterans experiencing or at risk of homelessness.

Offers a one-time budgeting and financial management workshop. Counselors can assist with apartment searching, applying to subsidized or public housing, and communicating with landlords. Depending on eligibility, can sometimes offer rental assistance to prevent or resolve a housing crisis.

Provides help with obtaining food and clothing, government ID's, assistance to see a medical professional, enter a drug or alcohol rehab program and assistance with housing. Serves dinner every night.

Assists individuals and families with securing and/or keeping stable, permanent housing. Services include homeless prevention, housing case management, emergency shelter in select circumstances. Also has a small food pantry (can serve anyone but focuses on items appropriate for homeless individuals).

Provides support to individuals and families who are at risk of experiencing homelessness, housing instability, and/or food insecurity. Provides a day shelter providing an immediate and temporary place for individuals and families experiencing homelessness to stay during the day hours while they seek to regain permanent housing stability.

Assists individuals/families with maintaining their home in the face of possible homelessness through a rental match program, budgeting help and help working with landlords. Program provides case management as well as the following classes: Money Management, Life Skills, Parenting, co-dependency training, Resume preparation, Financial match program.

Emergency service vouchers include: - Interfaith Food Pantry - Home heating fuel for seniors (age 60+) Dependent on availability of funding. - Prescription Assistance - Eye exams and glasses - Legal identification documents - Second hand furniture (Easton residents only) - Rental assistance: One month of rent. Documentation of income, circumstances leading to income lose or increase in expenses and proof of sustainability (Operates on a waiting list) Dependent on availability of funding.

Housing Case Management Services helps low income families find affordable housing with housing referrals and counseling on budgeting and tenancy readiness. Limited funds available for rental assistance on a case by case basis for their clients.

Provides housing and case management for families who are actively working with Bucks County Children and Youth Social Service Agency.
