Services include: Advocacy of Individual rights Development and maintenance of an Individual Support Plan (ISP). Determining the need for services and developing "outcomes." Identifying and locating providers of services. Coordinating waiver services when funding is available. Ensuring services are received as outlined in the ISP. Ensuring health and safety.

Community Integration is a service for those individuals who will be transitioning back into their community, or to a new community from a nursing facility or similar.

Provides activities and workshops for older teens and young adults with any disability.

Provides core independent living services, including information and referral, individual and systemic advocacy, peer support (in-person and virtually), and independent living skills training. Also provides transition services to support persons transitioning from school to the adult world, and from care facilities into the community.

Offers a wide spectrum of programs and services to meet the needs of the local disability community. Services include: *Disability Advocacy *Person Centered Counseling *Voter rights *Peer Support *E-sports *Technology Center

Enables persons with disabilities and their families to find, utilize, and coordinate available resources and opportunities in their communities based on their individual needs.

Provides skills training classes, peer support, advocacy, information and referral, nursing home, and youth transition services to individuals with disabilities.

Provides social gatherings and travel opportunities to individuals with disabilities. Events include dances, theater, summer camps, bingo, and trips ranging from local overnight trips to destinations that include Florida, Mexico, Punta Cana, and cruises.

Provides a comprehensive array of services and supports to children and adults with intellectual disabilities, including support coordination, in-home supportive services, out of home placement assistance, groups homes, and information and referral services.

Variety Club Camp and Developmental Center offers a wide array of programs and services for children with disabilities. Including after school care, Saturday and/or weekend respite, summer programs, and sports and aquatics programs

Provides professionally trained service and companion dogs to people with mobility impairments, seizure disorders, cardiac conditions (Dysautonomia), Diabetes, and other disabilities/needs.

Coordinates and administers services and resources to help older adults and individuals with disabilities to make decisions and choices about their own care and enable them to be active in their communities. Most services are delivered through the county area agencies on aging.

Provides advocacy for individuals with disabilities, independent skills training, and information and referral.

Provides home modifications to individuals with low-income. Offers ADA & UCC Assistance, Comprehensive, on-site assessments, technical assistance, and training on the federal and state anti-discrimination and accessibility laws. Gives training, Individualized, interactive workshops on disability awareness and disability-related issues, and monthly consumer training series.

Provides education and outreach presentations to businesses, provider agencies, community partners and people with disabilities. Presentations include Accessibility Surveys, Disability Sensitivity and Awareness, and guidance to assure compliance with the ADA.

Offers support and promotes independence of individuals with developmental disabilities. Provides information regarding resources and services available, assists with finding activities within the community and provides referrals to appropriate agencies within the community.

Provides services for Information and Referral, Case and Care Management, Benefits and Services Assistance, Advocacy, and Protection and Advocacy for Individuals with Disabilities.

Advocates and volunteers work on state and local disability issues.

Offers a two-week summer arts camp that hosts both neurodiverse and neurotypical learners for a series of classes in writing, visual arts, animation, music, and more.

Provides free training, information, and technical assistance to help parents throughout Pennsylvania become more effective advocates for their children. Services include information about the special education and healthcare systems, workshops and conferences on disability issues, and one to one technical assistance.

Delivers ready-to-heat, freshly prepared, nutritious meals to individuals at home that may not be able to access food otherwise. Meals prepared by chefs at LECOM Senior Living Center.

Improves access to supports and resources for long-term care through a network of local partners.

Provides counseling and advocates for individuals with disabilities seeking affordable and accessible housing.

Services for people with disabilities: housing referral service; technical assistance and recommendations for making existing and new structures accessible; care counseling and attendant care; homemaker services (in Washington County only); in-home skills
