Osteoporosis is a gradual thinning and weakening of bones, which can lead to bone fractures. More than 10 million Americans, 80 percent of whom are women, have or are at risk of developing this condition. Since osteoporosis is often referred to as a "silent disease," many are less likely to find out they have it until they suffer a painful bone fracture. Offers free tax help to people who generally make $60,000 or less, persons with disabilities, the elderly ,and limited English-speaking taxpayers who need assistance in preparing their own tax returns. IRS-certified volunteers provide free basic income tax return preparation with electronic filing to qualified individuals.

Participants work with a care manager, engage with a Certified Recovery Specialist, and attend outsourced behavioral/mental health appointments.

Provides adult day programs, including health care, nursing, social and recreational as well as occupational and speech therapies. May provide medication management, dental, transportation, optometry and respite. Services may be provided in the home.
