Assists with finding housing and making connections between individuals/families that have varying disabilities and property owner/managers. Connects people with disabilities to independent housing options of their choice, in the community of their choice. Serves as a resource of information, light case management, advocacy, and assistance with navigating through various housing systems.

Matches an individual with an intellectual developmental disability to a staff person of comparable age and interests; the staff works with the individual to foster a connection with the community. Participants become involved in activities such as concerts, shopping, going out to eat, etc.

Supports adults with intellectual disabilities in daily living, recreation, and education. Provides instruction in health and safety, socialization, daily living, recreation, medical needs and community access, and education about physical, occupational, and nutritional needs.

Provides emergency relief for bills and rent to prevent termination or eviction as well as offers assistance for security deposits. Can also help with car repairs by purchasing replacement car parts (up to $250), but will not cover the cost of labor.

Provides congregate meals, socialization, and enrichment/recreational activities including: arts and crafts, computers, bingo, cards, pool, puzzles, games, and exercise classes.

Provides 24-hour personalized care and special services to residents based on their unique individual needs. Offers a range of long- and short-term skilled nursing care services, including rehabilitation therapy, care for people with Alzheimer's Disease and other forms of memory impairment, respite, and hospice services. Includes inpatient and outpatient rehabilitation, enabling people to stay in their home whenever possible, with transportation to and from outpatient therapy sessions when needed. In addition, if you are admitted for a respite stay, recuperative care, or short stay rehabilitation, our discharge planning support will help to ensure everything is ready for your return home.
