Areas addressed may include socialization and decision-making skills, basic banking and budgeting, meal preparation, assistance with community integration, transportation as well as health and safety issues.

Individuals may attend the program full or part time. This program offers individual and group community-based career exploration, vocational, recreational and social activities for individuals participating in a community integrated setting that reflects individual goals.

A bilingual, bi-cultural residential drug and alcohol re-education program for women. Treatment is based on principles of individual responsibility, mutual help, personal effort and community support. Program requires 2-1/2 years of in-house treatment, followed by 5 years of follow-up counseling. All treatment is provided by trained, qualified ex-addicts. As part of its community outreach, Hogar Crea will provide the following services to persons in need:

Requests for service should be submitted at least 1-2 weeks in advance so the director can establish a work plan for the residents. Only "manpower" is provided, the agency does not have access to a truck.

Provides home modifications to individuals with low-income. Offers ADA & UCC Assistance, Comprehensive, on-site assessments, technical assistance, and training on the federal and state anti-discrimination and accessibility laws. Gives training, Individualized, interactive workshops on disability awareness and disability-related issues, and monthly consumer training series.

Provides a smoking cessation program for pregnant smokers.

Assesses callers for their readiness to quit and provides counseling options and structured assistance. Provides active listening to callers, educational information, and referral to other healthcare professionals if needed, prescription advice for action, social reinforcement, skill training, and encouragement for expressions of thoughts and feelings.

Operates an outpatient primary medical care clinic for Veterans through the Lebanon VA. Offers primary care and annual physicals by appointment, preventive services, women's health services, routine lab work, EKGs, immunizations, medication review, tele-mental health and social work services, treatment of conditions such as diabetes, hypertension, respiratory disease, common cold, influenza, tobacco cessation and specialty referrals. Telehealth: As a convenience to our patients, some services are offered by electronic means. Individual or group appointments may be performed via video to the Lebanon VAMC or other medical facility. Also, various tests can now be performed at any one of the six outpatient clinics, with the analysis being performed at the Lebanon VAMC or other medical facility. Educational Services:
