Transitional, or Bridge Housing, provides a home for individuals and families who have experienced homelessness. Residents continue to receive individualized Case Management to set goals, access resources, maintain stability, and grow into a more independent lifestyle.
The ESG is broken down into two sections: Rapid Re-housing and Homelessness Prevention. This program provides rental assistance for eligible individuals/families when facing eviction and provides rapid re-housing for individuals/families who are literally homeless. This program runs on an 18-month cycle with renewals.

Wayne County residents in need of assistance to rapidly rehouse the household after homelessness. The goal is to enable households to move into permanent housing, either market rate or subsidized, and providing the household with the short-term rental assistance, housing relocation, and stabilization services needed to prevent a return to homelessness.

Emergency Shelter Clearinghouses
Provides services to veterans and their families who are homeless or at risk of homelessness and/or lacking the resources for housing stability. Through the development of a service plan, assistance is provided with rent, utilities, emergency support, moving expenses, education, transportation and/or child care. Also networks with Veterans Affairs, churches and community groups to provide additional supports, benefits and services for Veterans and their families.

Eviction Prevention Legal Assistance
Assists individuals in securing financial resources, applying for pertinent documents, searching for housing and applying for housing. In some cases, financial assistance such as first month's rent, a security deposit and/or basic household needs is also available through PATH.
