Program activities include, but are not limited to the following: Adaptive modifications such as ramps, lifts, door widening, visual door bells, audio phones, visual phone signalers, etc. that will improve the ability of a person with disabilities to enter and exit the home. Adaptive modifications, such as widening hallways, lowering the kitchen counters, enlarging bathrooms, or adding grab bars that will increase the ability of a person with disabilities to perform activities of daily living. Other adaptive modifications that will enable a person with a disability to remain in the home and prevent institutionalization.

Specialized in home modification needs. Installs and constructs Indoor and outdoor ramps, stair glides and bathroom modifications.

Provides assistance to low-moderate-income residents of Chester County, with permanent disabilities, in making their current residences more accessible. May be used for homeowners and renters.

Provides qualified homeowners with a low-cost option for home maintenance and home repairs that make their homes habitable, safer, and more accessible.

Collects, refurbishes, and distributes durable home medical equipment to the local community. Also collects and refurbishes used and surplus medical equipment for shipping to hundreds of clinics in developing nations.

Provides immunizations, STD/HIV testing, pregnancy testing, access to breast/prostate cancer screenings, monitoring of chronic diseases, and Tuberculosis testing when available. Lead screenings provided in certain situations, not by the clinic.

Offers home modification services for individuals with disabilities to allow them to remain in their own homes. Advocates on behalf of individuals with landlords regarding modifications, and assists with identifying and evaluating contractors and funding sources for modifications.

Assess and help to resolve barrier issues of inaccessibility or discrimination. Conduct disability awareness training for schools, businesses, hospitals and civic organizations.

Provides home modifications to help increase accessibility for older adults in Pennsylvania so they can remain independent and safe in their homes. Offers Certified Aging in Place Specialists (CAPS) that provide technical assistance, coordination, and consultation for accessible home modifications.

Helps connect those with needs for food, clothing, housing referrals, rent assistance, budget planning, transportation, tutoring, etc. with the appropriate resources.

Provides home repair and maintenance services for low-income older adults, single parents, veterans, and disabled homeowners to increase the safety, accessibility, and affordability of housing. Offers repair services for minor plumbing, painting, sheetrock, carpentry, handiwork, and other property maintenance. Assists with post-disaster cleanup.

Assists homeowners, including veterans, seniors, and disabled homeowners with minor exterior home repairs. Typical projects include exterior painting, accessibility modifications (ramps, handrails, etc.), fence replacement, deck and stair, and overgrown brush removal. WE DO NOT REPLACE/REPAIR ROOFS.

Program designs and constructs accessibility modifications for homeowners and tenants, making it possible for individuals with disabilities to remain in their own homes. Items such as wheelchair ramps, accessible bathrooms, lifts, stair glides, widened doors and hallways, and both hearing and visual accommodations are installed by the program. The program cannot perform repairs.

Provides services that focus on assisting older adult homeowners to continue living safely and with dignity in their home and community. Coordinates and facilitates home modifications, ramps, stair glides, bathroom modifications, small home repairs, interior/exterior painting, porch and step repair, weatherization services, and home safety kit installation.

Provides consultation, evaluation, and treatment for a variety of issues that can impact emotional health and well-being. Services and supporting programs offered at the Coatesville Medical Center include: Community Residential Care, Mental Health Outpatient Clinic, Inpatient Psychiatry, Post-Traumatic Stress Disorder treatment, Substance Abuse Treatment Drug/Alcohol, Domiciliary Care, and a Work Restoration program. We also provide outreach to homeless veterans.The Social Work program also offers counseling to veterans with drug, alcohol, and emotional problems, to help veterans and their families understand and adjust to an illness or disability, to veterans and family members who want to prepare advance directives and durable powers of attorney for health care or finances, to help veterans and their families cope with terminal illness,and to family members about legal issues such as commitment and guardianship.

Assists with the process in obtaining services that provide home modifications for individuals who would like to stay in their own home but require environmental changes to accommodate physical challenges. Home modification services are managed by CIL through a waiver system. A staff member is assigned to you to assist you in gaining access to needed services.
