Provides disease prevention and health promotion services at senior centers or other community sites. Prevention programs include: "Live Your Best Life" a free, evidence-based, chronic disease self-management program and "Live Your Best Life with Diabetes" is a free diabetes self-management program. Also, certified instructors present a free, evidence-based program on fall prevention. Medication reviews with a registered pharmacist from Evangelical Community Hospital are offered annually at each senior center and free blood pressure screenings are offered by licensed nurses at each senior center.
