HealthyCare Card (HCC) is a program of Healthy Community Network (HCN). HCN is a collaboration with Health care providers and social service agencies who are committed to working together to improve access to healthcare services for those in our region who are uninsured or under-insured. A completed application with proof of income and assets will determine what discounts a person may be eligible to receive.
