Health centers are the implementation sites for the Department of Health's public health programs. Services include communicable disease reporting and investigation; epidemiology, informational and referral; chronic disease prevention and intervention; clinical services (including sexually-transmitted disease and tuberculosis diagnosis and treatment, immunization, and HIV testing, counseling and education); family health programs and environmental health services (including animal bites and rabies information).

With an emphasis on treatment and prevention of the spread of HIV for adults, PinnacleHealth's Resource Education and Comprehensive Care for HIV (REACCH) Program provides primary medical care and HIV management for men, women and adolescents. This comprehensive care program, coordinated by a registered nurse, is comprised of diagnostic, therapeutic and supportive services including education about HIV disease and transmission, as well as treatment recommendations and how to access them. Medical care is managed by an infectious disease physician and a nurse practitioner. In addition, medical management includes: nutritional counseling social service support financial counseling and a link to community resources. Offers, screening for HIV as well as medical services for those living with HIV/AIDS. Currently offering in person services only by appointment. Telemedicine appointments are available.

Current Services: - Free HIV Rapid Testing (Oral mouth swab with results in 20 minutes) is provided without an appointment Monday through Thursday, 9:00am to 11:00am and 1:00pm-4:00pm, Friday, 9:00am to 11:00am - Education information on labs and medications related to the treatment of HIV/AIDS - Primary Medical Services - Referrals for Mental Health - Referrals for Drug and Alcohol Counseling - Nutritional Counseling - Social Work Case Management - Meets the collective needs for persons with HIV/AIDS. LGHP-CC is a Ryan White Federal grant site. - Four doctors on staff who specialize in HIV/AIDS and are board-certified Family Medicine Physicians.
Provides medical case management services to persons living with HIV/AIDS and persons at-risk for the disease, including food and nutrition assistance, transportation assistance, psychosocial and mental health resources, dental assistance, and housing assistance (HOPWA). Also provides HIV and STD testing and programming for prevention, education, and outreach throughout Montgomery County.
