You will live in a home with other young men. On your first day in the home, you will be introduced to your treatment team, to the other residents and to the daily routine of the home. You will meet privately with a professionally credentialed clinician who will listen to your concerns, help you identify areas in your life that you would like to change and help you develop a plan to make these changes.

Supports for Living provides services to over six hundred individuals with Intellectual and/or Developmental Disabilities (IDD) and intends to maximize growth and independence for all of the individuals we support. Elwyn offers residential services and supports for daily living for individuals with intellectual and developmental disabilities in Chester, Delaware and Philadelphia counties. Residential options include campus living, community-based homes and apartments, supported living arrangements, in-home supports, life sharing and respite opportunities. In addition to these living options, Supports for Living offers a number of specialized residences, designed to provide the additional supports needed by residents with medical conditions or specific syndromes. Provides residential treatment services to adults with behavioral health needs and intellectual disabilities. supervision of skills training, vocational/educational planning, psycho-social assessment, counseling and peer support in order to increase greater level of independence, interdependence, peer/family support and empowerment.

Provides a permanent home in four, two-bedroom apartments for people living with serious mental illness. Residents receive support including: medication management, therapeutic services, social interaction, support in maintaining a safe and healthy home, transportation, nutrition education, and service coordination. Residents maintain their own apartments with staff support.

Provides residential rehabilitation and supportive housing for individuals with mental illness. Individuals participate in developing their own treatment plan and receive comprehensive mental health services while living in a community setting.

Offers a residential program for adults with a diagnosis of serious mental illness. It is located within a 13 bed group home with staffing 24-hours a day, seven days a week and it provides residents with all their meals. The staff provide supportive counseling, personal care and grooming, and assistance with daily living tasks to residents and assistance with medications. The program empowers residents to make choices by focusing on their strengths.

From outpatient treatment to long-term residential care, our team supports you on your path to recovery. We offer individualized care for people with mild to severe mental health issues, including depression, anxiety and personality disorders. Mental Health Services include: Outpatient Treatment, psychiatric rehabilitation, peer support services, partial hospitalization services, residential programs, vocational services and case management.

Offers a variety of residential services based on level of need, including Community Living Arrangements (CLA's) with direct support up to 24 hours/day; Supported Living Arrangements option for individuals with developmental disabilities who are capable of living safely and well with 29 hours or less of direct staff support per week; and Lifesharing which offers an opportunity for a person with disabilities to live as a valued member of a family.

Supports and assists women who are survivors of Commercial Sexual Exploitation (CSE) or who were victims of human trafficking. Provides individual and group counseling to address social and psychological trauma; past sexual, physical, and mental abuse; posttraumatic stress disorder; and other issues.

Offers a multi-disciplinary evaluation and rapid stabilization diverts individuals from more costly and restrictive inpatient hospitalization. As a step-up/down solution, the RTF at Natale is also a viable option for individuals who no longer meet inpatient criteria, but who need further treatment before returning to community life.

Offers a therapeutic, community-based residential program for adults with developmental disabilities and behavioral issues. This highly structured program provides rehabilitation for adults in need of intensive clinical/forensic treatment.

Residential setting offering 24/7 supervision and supports to help young adults learn to manage the symptons of their mental health condition(s) through individual and group therapy while simultaneously learning the necessary skill required to live sucessfully and independently. Individuals are assisted to gain employment and independent housing prior to leaving the program after 18 to 24 months.

Provides a range of housing options for those with serious and persistent mental illness, including both short-term, long-term and independent housing. A) Community Residential Reabilitation (CRR) Recovery based setting that is transitional and rehabilitative in purpose. Residential staff members assist residents in building skills that are necessary to move to a more independent setting. Residences consist of a cluster of shared apartments located throughout the community and support is provided at a Maximum level. Staff members are available on an on-call basis during the overnight hours. (Serves 14 individuals.) B)Long Term Housing Programs Quarters B Residential staff members assist residents with recovery goals, providing support in a quiet country setting.
