Adult Residential Behavioral Health Program operates seven Personal Care Homes in Delaware County. All are licensed by the Pennsylvania Department of Human Services.

Adult Residential Treatment Facilities
Provides residential services including moderate-care community residential rehabilitation and supported living. Provides services and support by trained mental health professionals to assist residents in building skills of adult daily living and to gain greater independence, self-sufficiency, and self-confidence, as well as, to enhance overall recovery. A portion of the program is dedicated to serving Transition Age Youth (ages 18 to 26).

Offers a 15-bed residential treatment facility for people who require mental health treatment and services on an on-going, 24-hour basis for up to 30 days. Provides a safe and supportive environment as an alternative to inpatient mental health hospitalization or as a step-down from it. Services are individually tailored to meet the client's needs and can include individual and group therapy, psychiatric services, and medication management. Clients are invited to join daily recreational activities.

Mental Health Information/Education
Offers limited intensive psychiatric treatment and is designed to provide crisis stabilization and acute psychiatric symptom reduction as an alternative to inpatient psychiatric care. Clients served cannot benefit from traditional outpatient psychotherapy at the time of referral. Treatment is goal focused and assessment and treatment are reviewed on a daily basis. Discharge planning is an integral part of the program, with recipients leaving with arrangements for less intensive levels of treatment in place.
Supports for Living provides services to over six hundred individuals with Intellectual and/or Developmental Disabilities (IDD) and intends to maximize growth and independence for all of the individuals we support. The services provided by Supports for Living in a variety of residential settings and service programs promote individualized health care, social development, employment, recreation, community integration and other aspects of daily living. Elwyn offers residential services and supports for daily living for individuals with intellectual and developmental disabilities in Chester, Delaware and Philadelphia counties. Residential options include campus living, community-based homes and apartments, supported living arrangements, in-home supports, life sharing and respite opportunities. In addition to these living options, Supports for Living offers a number of specialized residences, designed to provide the additional supports needed by residents with medical conditions or specific syndromes. Provides residential treatment services to adults with behavioral health needs and intellectual disabilities. supervision of skills training, vocational/educational planning, psycho-social assessment, counseling and peer support in order to increase greater level of independence, interdependence, peer/family support and empowerment.

Residential Placement Services for People With Disabilities
Provides individuals who have shown a high level of skill but still require some assistance in living in the community. Provides up to eight hours per day supervision. Resident Advisers assist individuals in cooking, banking, shopping, doctor's appointments and can be available for any other questions or concerns that an individual may have.

Offers a multi-disciplinary evaluation and rapid stabilization diverts individuals from more costly and restrictive inpatient hospitalization. As a step-up/down solution, the RTF at Natale is also a viable option for individuals who no longer meet inpatient criteria, but who need further treatment before returning to community life.

Serving Delaware and Philadelphia Counties, Elwynâs community-based clinical wraparound services are individually designed to include multiple disciplines in an effort to address the specific and unique challenges of the child. A clinical team of behavioral health professionals, which may include a behavioral specialist, lead clinician, mobile therapist and therapeutic support staff who offer families and children tools to succeed within their least-restrictive educational and living environment.

Clients are encouraged to function as independently as possible via making decisions and choices on activities of daily living and community participation.

Provides a permanent home in four, two-bedroom apartments for people living with serious mental illness. Residents receive support including: medication management, therapeutic services, social interaction, support in maintaining a safe and healthy home, transportation, nutrition education, and service coordination. Residents maintain their own apartments with staff support.

Crisis Residential Program (CRP) at Natale provides a safe, structured setting where adult patients with urgent/emergency needs can receive crisis stabilization. The CRP at Natale provides interim rehabilitative care and support to individuals with mental illness as an alternative to more restrictive inpatient admissions.
Offers a range of mental health residential services depending on the need of the person. Programs options include a 60 to 90 day placement in Chartiers' RTP, an intensive mental health treatment in a residential setting. Referrals to the RTP helps the consumer avoid inpatient hospitalization and provides support while their symptoms stabilize. There is also a Long Term Structured Residence (LTSR) which offers a 16-bed capacity for specialized geriatric residential service. Chartiers Center's newest program is called Janus. It is for people with a mental health diagnosis and it provides scattered site apartment living. The goal is to help people move to independent housing. For more information call 412 221-3302.

Residential Placement Services for People With Disabilities
Provides a range of housing options for those with serious and persistent mental illness, including both short-term, long-term and independent housing. A) Community Residential Reabilitation (CRR) Recovery based setting that is transitional and rehabilitative in purpose. Residential staff members assist residents in building skills that are necessary to move to a more independent setting. Residences consist of a cluster of shared apartments located throughout the community and support is provided at a Maximum level. Staff members are available on an on-call basis during the overnight hours. (Serves 14 individuals.) B)Long Term Housing Programs Quarters B Residential staff members assist residents with recovery goals, providing support in a quiet country setting. Residents of this program are able to live in the program for as long as they wish, given residential guidelines. (Serves 5 individuals.)
