Provides services specifically designed for the seriously mentally ill. Many of these individuals have spent a significant amount of time in state institutions, struggle with drug and alcohol dependency, have multiple physical problems and have previously displayed violent behaviors. They have little or no social support, but seek new opportunities and better quality of life.

Provides community-based treatment and rehabilitation services that combines recovery with integrated approaches that address an individual’s psychiatric, medical, and behavioral health needs. Services include diagnostic assessment, crisis intervention, individual and group psychotherapy, medication evaluation and management, and mental health recovery. Support services include rehabilitation activities that help clients learn or improve the skills required to live, work, and learn in the community; and coordination and development of community supports to connect clients to essential services.

Peer specialists assists adults with mental health issues in basic living skills, support and advocacy, developing a social network and linking them to other services.

Provides consumers discharged from psychiatric hospitals and their families, with assistance and support accessing the large number of community services available to them. Significant aspects of the work include consulting with in-patient staff and assisting with discharge planning. Heavy emphasis is placed on linking discharged consumers into needed Consortium programs and other programs of the consumer's choice.

Treats adults by keeping them in their homes and communities. The psychiatrist-led team of clinicians provides assessment, medication management, crisis intervention, integrated case management, crisis intervention, integrated case management, and behavioral support planning to keep the individual from being placed in a state hospital or other long-term patient setting. Maximum length of stay of 18 months with a goal of achieving results within 12 months.

Provides peer specialists who have been trained and have the desire to partner with mental health consumers to help them gain strength, support and hope in their road to recovery.

Provides a program that assists individuals with bridging the gaps between their needs and available resources. Community Health Workers can meet with clients in a variety of locations to determine the current barriers that are impacting an individual's overall health and wellness. Community Health Workers will help individuals to create a plan with goals and action steps address these barriers.

Provides mobile peer support services offered by certified peer specialists who have lived with and experienced mental health challenges. Services do not replace traditional therapies, but instead offer an alternative to individuals who may feel alienated and disconnected from the mental health system.
