Offers connection to a Certified Peer Specialist (CPS), who is an individual with lived mental health experience who has been trained and certified in the state of Pennsylvania on how to assist others in recovery and wellness. Certified Peer Specialists can: A) Assist in the development of strengths-based individual goals B) Serve as an advocate, mentor, or facilitator for resolution of issues that a peer is unable to resolve on their own C) Help to develop community supports D) Provide education on ways to maintain personal wellness and recovery E) Provide education on navigating the mental health system

Support services include rehabilitation activities that help improve the skills required to live, work, and learn in the community; and coordination and development of community supports to connect to essential services.

Supported housing for persons who suffer from chronic mental illness. Live independently in the community while receiving the supportive assistance they need through regular in-home visits. Delivers customized services to help people with disabilities accomplish daily tasks. People in this program don’t require 24/7 support; instead, they pursue interest-based activities and develop life skills

Case management services for adults with behavioral health issues identified as serious mental illness. Case managers develop recovery plans for individual clients, arrange for required services, monitor services and serve as an advocate for the individual. Offers mental health support services including information/education, intervention, self management and transitional services. Also provides post disaster mental health services.

Offers services that help individuals living with mental health challenges to increase self-esteem, build community supports, identify community resources, and learn about symptom management and coping skills.

Provides peer specialists who have been trained and have the desire to partner with mental health consumers to help them gain strength, support and hope in their road to recovery.

Provides consumers discharged from psychiatric hospitals and their families, with assistance and support accessing the large number of community services available to them. Significant aspects of the work include consulting with in-patient staff and assisting with discharge planning. Heavy emphasis is placed on linking discharged consumers into needed Consortium programs and other programs of the consumer's choice.

The staff will help you identify areas of need through an assessment period and will help you set, achieve, and maintain a goal. Staff will provide ongoing knowledge and awareness, support and skills training; but you are the one in control of your own recovery journey.
