Relapse Prevention Program is for those at risk for chronic relapse. Patients receive individual and small group therapy to help them address the underlying issues that drive them to repeated patterns of self sabotage and relapse.

Screens clients to determine treatment/non-treatment needs. Provide supportive counseling and assistance with referrals. Provide re-entry services for the substance abusing/dependent client who is transitioning from incarceration or fulfilling criminal justice mandates. Monitor drug/alcohol use by performing breathalyzer tests and urine drug screens and report the results to the assigned probation officer or designee of the Court. Provide case management services to clients who have been in the criminal justice system. Case management provides linkage to treatment facilities if identified; recovery support services including; tangible needs (food, clothing); community resources (self-help groups); various housing needs; education; employment and training; and medical care.

Offers supportive, sober environment where individuals with substance use disorders may participate in social activities and connect with peers. Information and referral for various social service agencies, including 12-step programs, also available.

Provides Case management to help individuals who struggle with substance abuse develop a sober and drug-free lifestyle, while working towards stability in housing, education, employment, transportation, child care, and healthcare. Services include: connecting individuals to treatment options; assisting with application processes such as Medical Assistance, Social Security, job applications and educational opportunities; legal advocacy; life skills coaching; referrals for transportation, childcare, food, clothing, and other basic needs.

Navigates clients through life's difficulties, and shares the skills necessary to live successfully and remain in sustained recovery.

Through a series of information workshops, we will build skills, set goals, and gain the tools needed to maximize recovery capital and find fulfillment in a chosen career path.
