Coordinates living arrangements with contracted families who provide the supervision and services to the consumer. Families are paid a monthly stipend by the provider agency for the service. Living arrangements are made based on each situation. Consumers can live in a family's home, families can live in a consumer's home, or a provider agency can broker a mutually satisfactory living arrangement.

The Supplemental Nutrition Assistance Program is the new name for the Food Stamp program. These benefits are used to buy food and help eligible low-income households in Pennsylvania obtain more nutritious diets by increasing their food purchasing power at grocery stores and supermarkets. Provides replacement SNAP benefits to purchase food that was lost due to power outage, flood, or fire. Must apply within 10 days of disaster. Fill out and submit replacement affidavit.

Provides Stay and Play that is an afterschool program. Housed at the Zion Lutheran Church, it is a year-round program that seeks to enrich the lives of children through tutoring, Science, Technology, Engineering, Arts, and Math (STEAM) activities, positive social interaction with peers and adults. Uses educational games, art projects, and quality role models to engage the children.

Offers full service hospice program providing physical care by nurses and home health aides. Assists with home, family or legal issues by a medical social worker. Offers emotional and spiritual support from a spiritual care counselor, practical help from trained volunteers and follow-up bereavement support for caregiver and family for a period of one year.

Cafeteria style meals served free of charge to those in need. September through April only

A prenatal services program which integrates comprehensive and culturally sensitive clinical care with care coordination and social work services, nutritional counseling, family planning referrals, WIC referrals, EPSDT referrals, dental screening, prenatal and childbirth education and other related services.

Offers a comprehensive team approach to mental health treatment and service coordination for individuals with behavioral health challenges who have not progressed in their recovery after receiving more traditional behavioral health services.
