The Family Center Program is a home-visiting service for pregnant women and caregivers with children ages 0-5 years old. Using the evidence-based Parents as Teachers curriculum, trained professionals visit families’ homes to provide personalized education and support focused on healthy pregnancies, positive parenting, and early childhood development. The program helps identify and address child developmental delays, parental stressors, and a wide range of family needs, including helping families meet their basic needs. The core focus is on child development education and ensuring children are well-prepared when they enter the K-12 system.

This program is provided for children with developmental delays and is designed to teach parents how to address a child's delays in their natural environment. Speech, physical, and occupational therapy are also provided.

Provides a variety of need-based services for eligible young children, including: consultations, instruction in specialized classrooms, related services, for example in the areas of speech, hearing, and vision, and assistance with psychological evaluations and assistive technology in addition, provides screenings, upon request, and resources, training, and outreach programs for schools, preschools, colleges, and various agencies.

A Supports Coordinator (case manager) works with each individual to develop and Individualized Service Plan (ISP) focused on what the individual needs to meet their maximum potential. Supports Coordination is available at no cost to registrants.

Provides services and supports designed to help families with children who have developmental delays or disabilities. Services can include: information about how children develop; parent or caregiver education; family supports; and developmental and instructional therapies. Early Intervention is individualized and provides supports to the child and family in the home or in the community.

Families partner with their service coordinator and EI providers to build a plan based on their child and family's unique strengths and needs. Through coaching, families learn how to build opportunities for learning into their unique daily routines and activities, at home and in the community, The team providing services may include one or more of the following based on the family's individual needs: a teacher, speech, physical, and occupational therapists, vision and hearing specialists, behavior specialist and a list of other services.

Birth to age 3 who have developmental delays or conditions which have a high probability of delays.

Offers free linguistically and culturally-appropriate health services, education, and social services to pregnant women, children ages 0 to 3, and their families. Services include weekly home visits providing education and support services during pregnancy through baby's third birthday; education and family support services for families with eligible children at state licensed child care center; services to children with disabilities; and connections to resources for healthcare, housing, and emergency supplies.

Family centered program that provides services and supports to families who have children with developmental delays. Builds upon the natural learning that occurs in the first few years of life. All services are provided in the child's most natural environment.

Home and community based services and supports are provided to families with infants and toddlers to maximize the child's developmental potential and can assist the family in helping the child develop and grow. Service planning and service delivery is founded on a partnership between families and early intervention personnel, which is focused on meeting the unique needs of the child and family.

Provides early intervention services to children experiencing disabilities or delays.

Provides early intervention services to children. Qualifying children may be eligible to receive physical therapy, occupational therapy, speech therapy, hearing therapy, vision therapy, and special instruction. Each child is provided with a Service Coordinator who can be a resource and support for the child and family. All services are provided in families’ homes or the community.

Provides support and services to meet developmental needs. Offers services that provide skills to children and families related to the child's development, such as communication (speech and language) and social/emotional.

Consists of services and supports designed to help families with children who have developmental delays. Services may include: information on how children develop skills and abilities; education and support for the parent/caregiver, and specific developmental therapies designed for their child.

Provides early intervention services for children who have shown a delay in mental or physical developmental areas.

Provides home based therapy and parenting education for parents of children 0-3 with hearing loss and speech/language issues.

Identifies strengths and potential developmental deficits of children in the program and also guides the Child Development staff in the individual and group curriculum they provide to the children while their mothers are in treatment. Also provides age appropriate activities and parenting tips for the mothers in the program.

Designed to meet the developmental needs of children from birth to 3 years old with a developmental delay or disability as well as the needs of the family related to enhancing the child's development. Provided at no cost to families and is designed as a family centered approach. Supports and services to families and their children are individually based upon strengths, concerns, and priorities of families and their child's needs. Services should be conducted in the child's natural environment. Also assist families to access many community services and supports in their local area.

Offers early intervention programs designed to help children overcome areas of delayed development. Advocates to ensure they receive appropriate special educational services such as speech therapy, occupational and physical therapy, and other special instruction.

Teachers and/or therapists provide this service to infants and young children in their own home or in a day care setting. The family-based program is available to children living in Wayne, Lackawanna and Susquehanna counties. The process begins with evaluations. Program plans are then devised with input from county personnel, parents and Center staff. During scheduled sessions, the special-needs instructor/therapist works with the child and his or her family on routine-based activities that foster developmental growth.

Early Intervention supports are designed to meet the developmental needs of children with a disability as well as the needs of the family in relation to the child's cognitive, communication, social, emotional, and adaptive development.

Early intervention is an array of supports and services that promote opportunities for parents to enhance their capacity in raising their child with developmental delays/disabilities. Specifically, parents/caregivers are taught how to support their child to help him/her grow. Early intervention services could include, but are not limited to: Special Instruction, Assistive Technology, Speech and Language, Audiology Services, Family Training and Counseling, Occupational and Physical Therapy, Psychological Services, and Vision Services.
