Offers a nutritious meal created to address the specific health conditions of the senior, a safety check around the house and an assessment of any changes that might impact future medical needs.

Partners with 35 different houses of worship that offer either sit-down community meals or "to go" meals. AHTN volunteer drivers will either pick up our homeless friends and bring them to the sit-down community meal or deliver "to go" meals to our homeless friends at various bus stops, depending on the house of worship preparing the meal. Delivers meals to the various bus stops that are AHTN's usual pick up spots in Lower Bucks.

Delivers ready-to-heat, freshly prepared, nutritious meals to individuals at home that may not be able to access food otherwise. Meals prepared by chefs at LECOM Senior Living Center.

Offers short-term services to older adults who are in need of assistance to remain or regain their independence in their home. Services include adult day care services, CCT Transportation, home delivered meals, companionship, respite care, errands, meal preparation, light housekeeping, personal care, incontinence supplies, nutritional supplements, and more.

Offers an extensive life-long learning curriculum, a vibrant arts and humanities program, and a rewarding intergenerational program. Offers a wide array of trip opportunities and evidence-based health promotion and wellness programs, including an on-site fitness center. Center programs also offer diverse groups of people opportunities to connect and to better understand each other.

Home delivered meals.

Meals on Wheels is a voluntary non-profit organization. Provides one hot meal and sandwich meal daily, Monday-Friday (except holidays), all low salt diets. Cancellations should be called the day before.

Provides home and community-based services to enable consumers to remain in their home. Services are provided on a sliding-fee scale to those who qualify. In order to participate, an individual must be referred to and assessed by the department. After the assessment, a care manager will be assigned to develop a plan of care that meets the needs of the individual.

Offers online searchable directory of meal delivery programs for seniors nationwide. Users can search by ZIP code to get listings of local agencies that offer home delivered meals.

Provides a home-delivered meal to those who are unable to prepare their own food because of physical limitations. Also provides a check in by a volunteer.

Providing a fleet of volunteers drivers to deliver FED Boxes with a variety of food items including fresh produce, canned products, and protein items. FED boxes are monthly emergency food delivery.

Offers daily meal. Offers brief contact through the volunteer delivery. Serves as a well-being check for the home-bound individual.

We deliver meals every day Monday thru Friday

Program is for clients ages 18 to 59 years who are disabled and home bound and are in need of food. If they meet the program guidelines, then they would be eligible for 5 free meals each week.

Provides meals to the homebound.

Offers the following Senior Center Services: Recreation, socialization & education; Information & referral to area agency on aging caseworker-approved services; Specialized transportation; Volunteer opportunities; Monthly minimal health screening; Weekday noon-time meals; Home-delivered meals for home-bound unable to prepare their own; Tax service (fee by donation); and Transportation can be provided through Rabbittransit. Operates as a cooling station for those who cannot adequately cool their homes or do not have a place to cool down.

Provides freshly prepared meals at a reasonable cost to anyone in their specified geographic area who cannot reasonably provide meals for themselves.

Provides independent living services to older county residents such as home delivered meals, transportation, emergency/legal assistance, SHARE Housing, senior volunteer program, and case coordination.

Provides home delivered meals to eligible older adults. Coordinates home delivery of meals during the week with help from dedicated volunteers and community partners including Main Line, Havertown and Media area Meals on Wheels; the Chester County Department of Aging Services; as well as the Delaware County Office on Aging (COSA).

Provides basic needs and emergency resources such as furniture, food delivered weekly, clothing, toiletries, and information.

Provides an act of pastoral care to those recently discharged from a hospital or nursing home by delivering a gift of home made soup from the parish.
