Meals on Wheels for Schuylkill County residents between the ages of 18-59.

Provides a comprehensive listing of Meals on Wheels services in Westmoreland County.

Provides meals to seniors, individuals recently discharged from a hospital, young adults, and their pets. Services include delivery to homes along with a visit from a friendly volunteer. Offer special-needs menus for those with particular dietary needs. Welcomes volunteers and donations.

Provides a home-delivered meal to those who are unable to prepare their own food because of physical limitations. Also provides a check in by a volunteer.

Provides freshly prepared meals at a reasonable cost to anyone in their specified geographic area who cannot reasonably provide meals for themselves.

Delivers hot and cold meals to individuals with disabilities and/or a limited ability to leave home. Able to accommodate special diets such as diabetic, low salt, and soft.

Provides a well-balanced hot lunch meal for delivery.

Provides a home-delivered meal to those who are unable to prepare their own food because of physical limitations. Also provides a check in by a volunteer.

Offers short-term services to older adults who are in need of assistance to remain or regain their independence in their home. Services include adult day care services, CCT Transportation, home delivered meals, companionship, respite care, errands, meal preparation, light housekeeping, personal care, incontinence supplies, nutritional supplements, and more.

Provides food; limited home delivery is available on a case-by-case basis. Limits assistance to once per month.

Offers the following services: A congregate meal program; Home-delivered meals available for those eligible; Annual nutrition screening and education; Regularly scheduled social, recreational, physical fitness, and educational programming; Online computers and classes are available at all of the activity centers; Transportation is provided by: Rabbittransit Senior Ride Program; Assistance in filling out forms and applications; Tax preparation services are available; Groups for chorus, bible study, art classes, book club, etc.

Services may include Care Management, Assessment, Protective Services, Information and Assistance, PA MEDI (Insurance Counseling), Health and Wellness Programs, adult day services, in home meals, personal emergency response system, personal care, transportation, and caregiver support. The agency also has 5 Senior Centers throughout the County in which participants can enjoy a Congregate Meal and Programming.

Healthy and Nutritious Meals prepared and delivered to home bound Older Adults.

Serves people who are at acute nutritional risk due to serious illness. Delivers 21 meals each week personally tailored to each client’s medical needs to help nourish them through the debilitating effects of their illness. Offers both individual and group nutrition counseling. Program is a temporary service based on individual needs assessment.

Provides home and community-based services to enable consumers to remain in their home. Services are provided on a sliding-fee scale to those who qualify. In order to participate, an individual must be referred to and assessed by the department. After the assessment, a care manager will be assigned to develop a plan of care that meets the needs of the individual.

Program that prepares and delivers regular meals to older adults and people with disabilities who are unable to shop and/or prepare the food for themselves or travel to a site where a meal is being served.
