Services provided to mental health consumers which enable them to access and retain permanent housing through suitable support, guidance, and assistance.

For chronically homeless individuals with disabilities or disabling conditions. Program openings are filled through the Coordinated Entry System.

Provides rapid rehousing, supportive housing, housing navigation, and related services to improve the health of homeless people living with HIV. Also assists people with criminal backgrounds in finding housing. Provides representative payee services to individuals who experience housing instability but may not need the additional supports of the housing program. Additional service includes medical transportation, advocacy, empowerment counseling, life skills training, document procurement, housing procurement, nutrition assistance and various other supports. In-person AND virtual options.

FCSDC partners with the Office of Behavioral Health and the Delaware County Housing Authority to provide housing vouchers to persons experiencing homelessness. Through various housing processes with the Continuum of Care and Coordinated Entry, homeless singles and families are identified and referred to FCSDC to identify safe and affordable housing.

Offers transitional housing program and a permanent supportive housing program for individuals and families experiencing homelessness in Butler County.

Provides affordable housing for low to moderate income seniors and families. Apartments are located in Allentown, Bethlehem, Emmaus, Coopersburg, Easton and Palmer. Some apartments are 55 and over, some are 62 and over.

Multidisciplinary community treatment teams provide intensive, individual support to individuals diagnosed with a serious mental illness in both Lehigh and Northampton counties. Specific services include psychiatric assessment, medication education and monitoring, treatment for co-occurring disorders, behavioral therapy, psychosocial services and education. A separate, intensive/blended case management unit provides continuity of care for those individuals with serious mental illness residing in Lehigh and Northampton counties who no longer require the intensive level of multidisciplinary assertive community treatment team services. The Lehigh Valley ACT Permanent Supportive Housing program provides independent, scattered site housing funded through a McKinney-Vento grant to chronically homeless individuals with serious and persistent mental illness. Provides supportive permanent scattered site housing to chronically homeless and disabled individuals and families. Participants are encouraged to complete a 12 month service plan, participate in weekly life skills classes, maintain savings, obtain employment or participate in educational programs as applicable, complete volunteer service, maintain health counseling where applicable and participate in regular home visitation by casework staff.

Designed to assist mental health consumers experiencing chronic homelessness in securing housing. Aims to provide low-cost housing to mental health consumers in Westmoreland County as well as assistance through Housing Supports. The PSH-PSH community living building has living space for 8 tenants.

Assist street homeless men and women to connect to community resources as they strive to move from homelessness to self-sufficiency. * Social Security Representative Payee Program: Provides financial monitoring and fund administration to individuals receiving social security who are unable to manage their own finances.

Intervention that combines affordable housing assistance with voluntary support services to address the needs of chronically homeless people. Referrals for services are made through Coordinated Entry System.

The St. Ann's Program in Hazleton secures four 2-bedroom apartments and four 3-bedroom apartments, for occupation by persons with a qualifying disability, along with their families.

A permanent supportive housing program for homeless women. The maximum length of stay at Liberty House is 24 months. Client residents attend counseling and life skills education workshops. The staff work with participants on money management, parenting, job hunting, and nutrition, with the goal of self-sufficiency.
