Provides permanent supportive housing assistance. Housing is provided at various apartment locations. Services include: Emergency Shelter, Permanent Supported Housing, Case Management, Life Skills Training , Outreach and Advocacy, Peer Support , Vocational/Educational Support , Participant Savings Plan.

Assist street homeless men and women to connect to community resources as they strive to move from homelessness to self-sufficiency. * Social Security Representative Payee Program: Provides financial monitoring and fund administration to individuals receiving social security who are unable to manage their own finances.

Provides rental assistance and weekly case management. Individual pays a portion of rent based on income, program pays the difference. Program covers security deposit and utilities.

Multidisciplinary community treatment teams provide intensive, individual support to individuals diagnosed with a serious mental illness in both Lehigh and Northampton counties. Specific services include psychiatric assessment, medication education and monitoring, treatment for co-occurring disorders, behavioral therapy, psychosocial services and education. A separate, intensive/blended case management unit provides continuity of care for those individuals with serious mental illness residing in Lehigh and Northampton counties who no longer require the intensive level of multidisciplinary assertive community treatment team services. The Lehigh Valley ACT Permanent Supportive Housing program provides independent, scattered site housing funded through a McKinney-Vento grant to chronically homeless individuals with serious and persistent mental illness. Designed to assist mental health consumers experiencing chronic homelessness in securing housing. Aims to provide low-cost housing to mental health consumers in Westmoreland County as well as assistance through Housing Supports. The PSH-PSH community living building has living space for 8 tenants.

Provides representative payee services to individuals who experience housing instability but may not need the additional supports of the housing program. Additional service includes medical transportation, advocacy, empowerment counseling, life skills training, document procurement, housing procurement, nutrition assistance and various other supports. Participants are encouraged to complete a 12 month service plan, participate in weekly life skills classes, maintain savings, obtain employment or participate in educational programs as applicable, complete volunteer service, maintain health counseling where applicable and participate in regular home visitation by casework staff.

Partners with the Office of Behavioral Health and the Delaware County Housing Authority to provide housing vouchers to persons experiencing homelessness. Through various housing processes with the Continuum of Care and Coordinated Entry, homeless singles and families are identified and referred to FCSDC to identify safe and affordable housing.

To provide decent, safe and sanitary housing for eligible low-income individuals, families and senior citizens. Provides information about eligibility for and vacancies in subsidized housing properties in Northampton County jurisdiction like Bangor, Bath, Rosetto, and Nazareth to name a few. Eligibility criteria include income guidelines, age and/or disability status. Waiting lists exist for all programs. All programs and options available on website at www.northamptoncountyha.org. Administers the Section 8 program. Eligibility is determined by income and family size.

Nittany House I and II provides Permanent Supportive Housing for individuals and families who have experienced chronic homelessness with a documented disability. The Housing Support Case Manager provides opportunities for case management, goal setting, and referral and support to help clients get to a better place after experiencing chronic homelessness.

The St. Ann's Program in Hazleton secures four 2-bedroom apartments and four 3-bedroom apartments, for occupation by persons with a qualifying disability, along with their families.
