Acts as the Coordinated Entry Agency for housing needs in Butler County. Potential services include emergency grants, permanent supportive housing for individuals and families with children, in particular those struggling with mental illness, and temporary/transitional housing and support

Provides eligible individuals permanent housing assistance through LCCAP.

Provides information about eligibility for and vacancies in subsidized housing properties in Northampton County jurisdiction like Bangor, Bath, Rosetto, and Nazareth to name a few. Administers the Section 8 program. Waiting lists exist for all programs.

Assists with resolving the disconnect between affordable housing and the needs and resources of low-income families. Provides supportive housing and expense assistance programs.

The St. Ann's Program in Hazleton secures four 2-bedroom apartments and four 3-bedroom apartments, for occupation by persons with a qualifying disability, along with their families.

Provides transitional housing for women and children with drug/alcohol and/or mental health disability. Provides a permanent housing program for homeless individuals or families with a drug/alcohol or mental health disability.

Connects clients with market-rate or subsidized units and offers life skills, wellness, employment readiness, and peer support through COLOURS’ in-house programs.

For chronically homeless individuals with disabilities or disabling conditions residing in Lackawanna County. Program openings are filled through the Coordinated Entry System.

Nittany House I and II provides Permanent Supportive Housing for individuals and families who have experienced chronic homelessness with a documented disability. The Housing Support Case Manager provides opportunities for case management, goal setting, and referral and support to help clients get to a better place after experiencing chronic homelessness.

Intervention that combines affordable housing assistance with voluntary support services to address the needs of chronically homeless people. Referrals for services are made through Coordinated Entry System.

Provides permanent housing for persons who are severely mentally ill, substance abusers, and the chronic homeless.

Provides permanent supportive housing.

Provides supportive permanent scattered site housing to chronically homeless and disabled individuals and families. Participants are encouraged to complete a 12 month service plan, participate in weekly life skills classes, maintain savings, obtain employment or participate in educational programs as applicable, complete volunteer service, maintain health counseling where applicable and participate in regular home visitation by casework staff.

* Coordinated Entry Access Site, connects individuals to programs within the Conference or refers out to community resources. Also helps qualifying low-income individuals and families with rental assistance, goal setting and budgeting. Provides bus passes for interviews or for first week of employment. Toiletries/personal grooming items provided as available. * Various rental assistance programs * Homeless Supportive Services: Provides intensive case management and referrals to other support services for single, adult, chronically homeless individuals. Assist street homeless men and women to connect to community resources as they strive to move from homelessness to self-sufficiency. * Social Security Representative Payee Program: Provides financial monitoring and fund administration to individuals receiving social security who are unable to manage their own finances.

Operates Permenant Supportive Housing through a HUD grant for chronically homeless individuals that have serious mental illness.

Partners with the Office of Behavioral Health and the Delaware County Housing Authority to provide housing vouchers to persons experiencing homelessness. Through various housing processes with the Continuum of Care and Coordinated Entry, homeless singles and families are identified and referred to FCSDC to identify safe and affordable housing.

Services include: Emergency Shelter, Permanent Supported Housing, Case Management, Life Skills Training , Outreach and Advocacy, Peer Support , Vocational/Educational Support , Participant Savings Plan.

Provides information about eligibility for and vacancies in subsidized and non-subsidized housing properties in Lehigh County. Eligibility criteria include income guidelines, age and/or disability status. Waiting list for all programs is 1 - 2 years. Administers the Section 8 program. Eligibility is determined by income and family size. Approximately 1-2 year waiting list.

Provides case management, housing opportunities, and permanent supportive housing for people with HIV/AIDS.
