Provides affordable housing for low to moderate income seniors and families. Apartments are located in Allentown, Bethlehem, Emmaus, Coopersburg, Easton and Palmer. Some apartments are 55 and over, some are 62 and over.

Nittany House I and II provides Permanent Supportive Housing for individuals and families who have experienced chronic homelessness with a documented disability. The Housing Support Case Manager provides opportunities for case management, goal setting, and referral and support to help clients get to a better place after experiencing chronic homelessness.

Provides decent, safe and sanitary housing for eligible low-income individuals, families and senior citizens. Provides information about eligibility for and vacancies in subsidized and non-subsidized housing properties in Lehigh County. Eligibility criteria include income guidelines, age and/or disability status. Waiting list for all programs is 1 - 2 years. Administers the Section 8 program. Eligibility is determined by income and family size. Approximately 1-2 year waiting list.

Provides a 6-12 month program that allows participants to strive toward independent living. Provides a safe and healthy environment to foster growth and promote change. Each participant’s journey is designed based on their specific goals and needs.

Acts as the Coordinated Entry Agency for housing needs in Butler County. Potential services include emergency grants, permanent supportive housing for individuals and families with children, in particular those struggling with mental illness, and temporary/transitional housing and support

Operates Permenant Supportive Housing through a HUD grant for chronically homeless individuals that have serious mental illness.

The St. Ann's Program in Hazleton secures four 2-bedroom apartments and four 3-bedroom apartments, for occupation by persons with a qualifying disability, along with their families.

Intervention that combines affordable housing assistance with voluntary support services to address the needs of chronically homeless people. Referrals for services are made through Coordinated Entry System.

Multidisciplinary community treatment teams provide intensive, individual support to individuals diagnosed with a serious mental illness in both Lehigh and Northampton counties. Specific services include psychiatric assessment, medication education and monitoring, treatment for co-occurring disorders, behavioral therapy, psychosocial services and education. A separate, intensive/blended case management unit provides continuity of care for those individuals with serious mental illness residing in Lehigh and Northampton counties who no longer require the intensive level of multidisciplinary assertive community treatment team services. The Lehigh Valley ACT Permanent Supportive Housing program provides independent, scattered site housing funded through a McKinney-Vento grant to chronically homeless individuals with serious and persistent mental illness. Provides case management, housing opportunities, and permanent supportive housing for people with HIV/AIDS.

Provides rental assistance and weekly case management. Individuals pay a portion of the rent based on income, program pays the difference, and covers security deposit and utilities.

Provides rapid rehousing, supportive housing, housing navigation, and related services to improve the health of homeless people living with HIV. Also assists people with criminal backgrounds in finding housing. Provides representative payee services to individuals who experience housing instability but may not need the additional supports of the housing program. Additional service includes medical transportation, advocacy, empowerment counseling, life skills training, document procurement, housing procurement, nutrition assistance and various other supports.
