Case/Care Management
* Coordinated Entry Access Site, connects individuals to programs within the Conference or refers out to community resources. Also helps qualifying low-income individuals and families with rental assistance, goal setting and budgeting. Provides bus passes for interviews or for first week of employment. Toiletries/personal grooming items provided as available.
* Various rental assistance programs
* Homeless Supportive Services: Provides intensive case management and referrals to other support services for single, adult, chronically homeless individuals. Assist street homeless men and women to connect to community resources as they strive to move from homelessness to self-sufficiency.
* Social Security Representative Payee Program: Provides financial monitoring and fund administration to individuals receiving social security who are unable to manage their own finances.
* Coordinated Entry Access Site, connects individuals to programs within the Conference or refers out to community resources. Homeless Permanent Supportive Housing
Multidisciplinary community treatment teams provide intensive, individual support to individuals diagnosed with a serious mental illness in both Lehigh and Northampton counties. Specific services include psychiatric assessment, medication education and monitoring, treatment for co-occurring disorders, behavioral therapy, psychosocial services and education.
A separate, intensive/blended case management unit provides continuity of care for those individuals with serious mental illness residing in Lehigh and Northampton counties who no longer require the intensive level of multidisciplinary assertive community treatment team services.

Partners with the Office of Behavioral Health and the Delaware County Housing Authority to provide housing vouchers to persons experiencing homelessness. Through various housing processes with the Continuum of Care and Coordinated Entry, homeless singles and families are identified and referred to FCSDC to identify safe and affordable housing.
