Provides in-home and community caregivers to support people of any age with physical, intellectual and/or developmental disabilities. Caregiver supports can be provided for a few hours a month, daily, weekly and/or 24-hour bases. Builds independence, as well as ensures the health and safety of the individuals that are supported.

Each resident is required to care for their own rooms and shared common spaces, actively participate in weekly house meetings, cooperatively manage household tasks, maintain at least part-time employment, cook for themselves, manage their own medication and medical appointments, and be willing to share a home with peers.

Provides community-based support to adults with intellectual and development disabilities. Offers a range of services including employment, day programs, and residential assistance.

Provides supported independent living in a shared housing environment. Residents are responsible for caring for their own apartment.

Provides intensive support to assist individuals to develop the practical skills they need to live as independently in the community as possible. Persons live together in a house with their own bedroom and shared living space. They are offered training in skills such as cooking, cleaning, budgeting, vocational and educational pursuits, time management, and medication management. Trained staff is available in the home 24 hours a day.

Provides counseling, therapy, and referral services; programs to address mental illness and developmental disorders; residential programs adults with mental illness or intellectual disabilities; and host home programs for children with emotional disturbances or intellectual disabilities.

Provides individuals with intellectual disabilities the opportunity to reside in the private homes of host families. Host families provide food, shelter, personal care, transportation, and comprehensive supports to one or two individuals.

Provides a supportive living program design to prevent psychiatric and medical re-hospitalization while giving individuals independent housing in a supportive environment. Provides a home-like living arrangement in the community for adults age 18 and older who need assistance with activities of daily living and are unable to live independently. Dom Care providers open up their homes to individuals who need supervision, support and encouragement in a family-like setting.

Provides subsidized housing options for older adults, individuals with disabilities, and those who qualify due to low income.

Offers supported living in the community and and 24-hour supervised residential living in licensed community homes for individuals with intellectual and developmental disabilities.

Offers community residential, respite, and life sharing services.

Provides Life Sharing and respite services to adults diagnosed with IDD in private residential homes.

Offers Supportive Living services provide support to individuals living in community apartments.

Provides semi-independent living in community homes that focus on mastering basic activities for daily living (meal planning, housekeeping, money handling, etc.), recreation and leisure activities (going to fairs, festivals, and other events), and offers an animal program (caring for dogs, cats, ducks, chickens, and other animals to teach teamwork and respect).

Provides residential options for individuals with developmental disabilities and/or for mental health consumers. Options for persons with developmental disabilities include: Community Homes, Supported Living Services, and Life Sharing. Options for Mental Health Consumers include: Personal Care Homes, Community Residential Rehabilitation (CRR), Supportive Housing: Permanent, Supervised Living, and Independent Living.

Provides counseling and advocates for individuals with disabilities seeking affordable and accessible housing.

Provides adult day programs and training, supportive employment services, children’s services-three approved private schools, medical rehabilitation, camp and respite programs, and residential and veterans’ services. The essential tools and resources provided enable individuals with disabilities to live their best life.

Provides community living for people with developmental disabilities through placement with a host family. Person-centered training for the foster family and the participating individual focuses on everyday living skills, family dynamics, and skill development.

Provides supports and services to help people live in their community as completely and independently as they desire.
