Matches adults who cannot live alone (because of age-related problems, or physical or developmental disabilities) with families who are willing to provide a supportive, family-like living arrangement for them. Dom Care providers care for no more than three people, and undergo a thorough vetting process. Also recruits and screens potential providers.

Provides supported, independent living in a shared housing environment. Each resident is required to care for their own rooms and shared common spaces, actively participate in weekly house meetings, cooperatively manage household tasks, maintain at least part-time employment, cook for themselves, manage their own medication and medical appointments, and be willing to share a home with peers.

Offers family-style residential homes, planned short term respite, home and community supports, companion services, life sharing, supported living, advocacy, volunteer opportunities, support for families and caregivers, shared lives, and spirituality.

Provides residential treatment for adults with behavioral health needs and intellectual disabilities, including skills training, vocational and educational planning, counseling, peer support, and assessments to promote independence and empowerment.

Matches adults or children with intellectual and/or developmental disability with a family/individual in the community to live together as a family unit, or in some cases companions. In addition to providing a home, the host family or companion (which are called providers), actively participates in supporting the person in cultivating self-care, social skills, and communication skills. The providers are also responsible for health and safety needs.

Provides subsidized housing options for older adults, individuals with disabilities, and those who qualify due to low income.

Provides an opportunity to share home and family with an individual with a developmental disability. Qualified families are selected and participants live with them in their residence. Families receive specialized training based on the participant's needs and at least 24 hours of annual training.

Provides community residential services, pre-vocational training services, and intermediate care for adults with intellectual disabilities.

Provides a home-like living arrangement in the community for adults age 18 and older who need assistance with activities of daily living and are unable to live independently. Dom Care providers open up their homes to individuals who need supervision, support and encouragement in a family-like setting.

Provides in-home and community caregivers to support people of any age with physical, intellectual and/or developmental disabilities. Caregiver supports can be provided for a few hours a month, daily, weekly and/or 24-hour bases. Builds independence, as well as ensures the health and safety of the individuals that are supported.

Offers support to individuals with intellectual disabilities to live with qualified unrelated adults who provide support in their home.

Provides a supportive living program design to prevent psychiatric and medical re-hospitalization while giving individuals independent housing in a supportive environment. Residents have access to 24/7 staff support. The program is designed to help individuals obtain independence of living within the community as well as aid in learning skills such as daily living skills, money management skills, and self-care. Also assists residents to work collaboratively with their medical professionals to understand their concerns and manage their medications.

Offers community residential, respite, and life sharing services.

Offers personal care services in the home, and domiciliary care for adults ages 60 and over.

Provides supports and services to help people live in their community as completely and independently as they desire.

Provides licensed homes in the community with 24/7 staff member support. Assistance is provided withmedications, meals, housekeeping, finances, decision-making, health and wellness, and emotional well-being. The Residential Program is offered on a long-term or a short-term basis. Supported Living program enables the individual to remain at home with support from staff to manage personal resources, health, mobility, transportation, and use of community resources to promote independence.

A supportive living relationship matching contracted provider(s) and individuals with similar lifestyles and interests; individual(s) live in the provider's home as a member of the family.

Offers residential homes and programs, supportive living, and life sharing.

Offers supported living in the community and and 24-hour supervised residential living in licensed community homes for individuals with intellectual and developmental disabilities.

Operates 23 Community Living Arrangements (CLA) and one Community Residential Rehabilitation home (CRRH).

Offers Intellectual Disabilities services, providing case management, family support services, community participation supports (formally called day programming), supported employment, residential habitation, in-home, and companion as well as respite.
