Prenatal/Postnatal Home Visitation Programs
Provides a program for new moms who are pregnant with their first baby. Once enrolled in the program you will be connected to a registered nurse who will provide the support, advice, and information to have a healthy pregnancy. Once the baby arrives, the NFP nurse will work with new moms on breastfeeding, nutrition, child development, safe sleep techniques, and much more.

Registered Nurses provide home visits to high risk pregnant women and/or parenting families, provide prevention education related to such topics as growth, development, nutrition and safety, perform screening related to post-partum depression, child development and child growth, visit families of children with such metabolic disorders as PKU or Maple Sugar Urine Disease, visit families of children with with cleft lip and/or palate and provide referrals and support for families that have experienced the loss of an infant ruled as Sudden Unexplained Infant Death (SUIDS).

Families meet monthly with community health workers to improve maternal and child health through education, advocacy, case management, and referrals to community-based services. Eligible participants may receive diapers, books, educational toys, and other household items.
