Also offers the Senior Health Specialist Program through which registered nurses provide health screenings and counseling, disease management education, nutrition education, and medication review for senior clients.

Provides a day shelter providing an immediate and temporary place for individuals and families experiencing homelessness to stay during the day hours while they seek to regain permanent housing stability.

Provides assistance necessary to keep seniors living independently in their homes and communities. Services include case management, employment services, work-readiness and job retention initiatives, computer literacy, peer mentoring, expungement clinics, anger management program, parenting classes, and more.

From CARP to in home case management and therapeutic services, Edison Court provides services to adults and youth in the communities in which they live.

Case managers assist individuals obtain supports that help reduce hospitalization so that they can continue to live in the community in the least restrictive setting possible. Offers recovery Certified Recovery Specialist (CRS) services. The CRS can assist clients in accessing various community resources such as education, housing, transportation, and recovery supports, as well as providing support over the phone and assistance in creating a personal recovery plan.

Offers effective prevention, education, intervention, and referral to treatment services to individuals and families in Blair county. Works closely with state, county and local agencies to prevent and reduce substance abuse. Works to address the devastating impact of substance use on the individual, their families and the community.

Provides assistance to address safety, permanency, and well-being issues of children. Includes child welfare, adoption and foster care, and other social services.

Provides access to service coordination resources for adults with disabilities who wish to receive attendant care services. Connects consumers with local agencies that provide personal care services, develops a service plan for consumers, and secures assistive technology, adaptive equipment or home modifications for select eligible consumers.

You and your family will meet privately with a professionally credentialed clinician who will listen to your concerns. The clinician will help you and your family identify areas in your life and ways you relate to each other that you would like to change. You, your family and the clinician will develop a plan to achieve these changes. The plan will usually include the clinician meeting regularly with your child individually as well as meeting with you as a family. You and your family will be supported by the clinician as you take actions to bring about these changes. The clinician will arrange for your child to see a PATH child psychiatrist, if indicated. You and your child will meet with the child psychiatrist who will complete an evaluation and may prescribe medication for your child, if indicated. You may continue to see both the child psychiatrist for medication appointments and see the clinician for therapy.

Offers trauma-informed psychotherapy for children, adults, couples, families, and groups emphasizing coping skills, emotion and behavior management, and recovery from mental illness. Provides Case Management and Resource Coordination services that assist individuals diagnosed with mental illness, and their families, by helping them access resources, identify personal goals and live their lives effectively and independently in their community.

Children and Youth staff provide assessment and casework services to families if a parent contacts the agency with concerns regarding their child who has committed a specific act or acts of habitual disobedience of the reasonable and lawful commands of his/her parent, guardian or custodian.

Provides access to drug and alcohol treatment services for uninsured individuals in Westmoreland County. Provides assessment of treatment needs, referral and linkage to needed services. Also provides case coordination and recovery support services for clients who have barriers to treatment services and recovery.

Assists with key resources such as obtaining a state ID, signing up for food stamps and medical insurance, providing transportation to and from appointments, and accompaniment throughout the entire process of navigating the shelter and housing systems.
