Provides assistance necessary to keep seniors living independently in their homes and communities. A regular schedule of visits and calls are scheduled to provide the necessary support, while also providing additional services during emergency situations such as severe weather, property damage, or utility issues.

Friends Association provides free legal representation, financial assistance, and social service support to individuals and families facing eviction through the Eviction Prevention Court Program. The program currently serves the Downingtown and Coatesville District Courts. Homelessness Prevention Programs provide community-based services and eviction prevention for families that balance financial assistance, landlord mediation, and case management for 12-18 months to support economic self-sufficiency and the resiliency of families.

Offers case management services to help individuals with health insurance and other benefit applications, basic needs supports via referrals, connections to behavioral health services, and health insurance education and care coordination.

Assures appropriate services are available for transition-age youth who are experiencing mental health challenges. Contracts with agencies who provide an array of services and resources which are focused around trauma informed principles, recovery, resilience, and self-determination. Services include high fidelity wraparound, outpatient treatment, blended case management, transition age housing, and program of assertive community treatment.

Children and Youth staff provide direct casework services to children and families dealing with issues of child abuse and neglect, along with the purchase, coordination, implementation, and evaluation of services for families in need of support.

Promotes self-sufficiency, self-determination, and stable community living among consumers of mental health services. Provides individuals with access to needed services and supports. Acts as an advocate on behalf of consumers. Includes two levels of Administrative Service Coordination (ASC) and Targeted Service Coordination (TSC). Administrative Service Coordination is available as needed for individuals who are managing community living on their own. Targeted Service Coordination is available to those who have more complex needs and challenges with community living.

Supports Coordination is the connecting bridge between the person with an intellectual disability/autism and the services and supports they need. Individuals with disabilities are assigned to a Supports Coordinator who locates, coordinates, and monitors services and supports for people with intellectual disabilities and autism.

Uses the evidence-based Parents as Teachers model to teach pre- and post-natal health education during home-visits in addition to providing health screenings and referrals for conditions like maternal depression and child developmental delays, health insurance and food benefits enrollment assistance, information on reproductive healthcare, family planning, childcare, and overall socio-emotional support for pregnant and parenting people with children ages 0-18 months.

The treatment team serves as the family's case manager. As such, the treatment team's responsibilities include assessment, planning, service linkage, advocacy, procuring goods and services, and referrals.

Develop, implement, assess and follow up on plans for the evaluation, treatment and/or care of people who need assistance in arranging services on an on-going basis.

Services include: supports coordination, information and referral, consultation and education, counseling, therapy, and family driven support services through various local institutions.

Offers effective prevention, education, intervention, and referral to treatment services to individuals and families in Blair county. Works closely with state, county and local agencies to prevent and reduce substance abuse. Works to address the devastating impact of substance use on the individual, their families and the community.

Provides drug and alcohol services to include prevention, intervention, outpatient, intensive outpatient, case management, recovery support and community based services. Also provides gambling outpatient treatment.

Directs prevention, intervention, treatment, and recovery support services for residents of Centre County and their families who are affected by the chronic disease of addiction. Case management staff provide outreach/education/referral services, and conduct assessments to connect individuals with drug & alcohol treatment. Intensive case coordination services are available for those individuals who need additional support in connecting to treatment services.

Offers transportation, education and specialized support services to meet the needs of qualifying individuals throughout the community.

Offers dedicated pregnancy and postpartum support services for individuals with Opioid Use Disorder (OUD) or Stimulant Use Disorder. Services include one-on-one peer recovery support, warm handoff referrals to Opioid and Stimulant Use Disorder treatment, care coordination among mental health, physical health, and pregnancy/postpartum health (i.e., obstetrics), and ongoing family recovery education. Provides comprehensive wraparound support for pregnant and parenting people.

Program that develops plans for the evaluation, treatment and/or care of individuals who, because of age, illness, disability or other difficulties, need assistance in planning and arranging for services; which assess the individual's needs; coordinate the delivery of needed services; ensure that services are obtained in accordance with the case plan; and follow up and monitor progress to ensure that services are having a beneficial impact on the individual.

Helps women assess their strengths and needs while guiding them as they think about their goals and how to reach them. Development of each woman’s service plan includes an assessment of immediate needs and her history and goals related to livelihood, housing, health and well being, family reunification, and criminal justice compliance.

Services include high fidelity wraparound, family based mental health, multisystemic therapy, outpatient treatment, blended case management, and mental health respite services.

Provides case management services to adults, children, and adolescents with a mental health diagnosis. Assists clients in locating services and resources to meet their needs and monitors the service delivery.

Meets with each resident and conducts a social history and assessment upon admission. When short-term residents are ready to return home, social services staff helps residents determine the best options for a safe discharge. This includes reliance on home health care, durable medical equipment, community services, and support groups.

Provides evaluation and recommendation a course of treatment related to the individual's strengths, needs, and goals.

Services include: supports coordination, information and referral, consultation and education, counseling, therapy, and family driven support services through various local institutions.

Provides specialized training and supports for both families and professionals who care for children and adults with challenging behaviors. Works with an individual and their care team to implement positive support strategies. Services can be provided at home, childcare, preschool, school, church, day program or residential living arrangement or in any setting in which supports are needed. Serve individuals aged 3 and older

Provides financial support and intensive case management to new refugees. Services include:

• finding them a safe and secure place to live and furnishing it with donated household goods and furniture
• greeting them at the airport and bringing them to their new home
• making sure they have a hot, home-cooked meal when they first arrive home
• helping them get Social Security cards and access public benefits
• ensuring they receive a thorough medical screening from one of the partner refugee health clinics at PENN Center for Primary Care for adults or Children’s Hospital of Philadelphia
• orienting them to the community and providing financial literacy lessons.
