Provides Early Intervention Service (EIS) coordination for Allegheny County families. Connects children in need with intervention services provided at their home or day care location.

Offers services for individuals with substance use disorders. Provides screening and level of care assessment for substance use disorder, referral to treatment, and support until admission if space is not available in an appropriate setting immediately. Care management includes connections to other human services (through warm hand-offs), information and education about treatment options, peer support, and coordinated linkage to treatment and/or services. Also offers a Safe Place to Stay providing food, shelter, and professional support while individuals are waiting for appropriate treatment placement in the community.

Provides those who are HIV positive with assistance in obtaining medical care, counseling, mental health treatment, substance abuse treatment, food and housing. Also provides prevention and education programs to high risk individuals and groups. Services may be provided at a client's home or by phone.

Offers a membership-based care coordination service for older adults who wish to live independently in their own homes. Provides a care coordinator who focuses on wellness and prevention and also provides case management during health crises.

Provide assistance to persons with serious mental illness who reside in homelessness situations that help them link with mental health treatment, rehabilitation, and housing resources.

Provides public health education, preventive nursing services, emergency preparedness, immunizations, communicable disease surveillance and investigation, HIV testing and counseling, community empowerment, and other nursing services as mandated by PA legislation. Provides adult and child immunizations, HIV/AIDS referrals, and health resources.

Provides case management services to adults and children who suffer from a serious mental illness or emotional disturbance. An assigned case manager works with the consumer and families to access, link, coordinate and monitor needed services as well as provide support and advocacy.

Offers short term Case/Care Management for families with children up to age 21 who are newly diagnosed with or are undergoing testing to diagnose any disability or health condition. Services are provided by Community Health Workers who specialize in health literacy, health education, and care coordination help to facilitate access to programs and providers.
