Offers services for individuals with substance use disorders. Provides screening and level of care assessment for substance use disorder, referral to treatment, and support until admission if space is not available in an appropriate setting immediately. Care management includes connections to other human services (through warm hand-offs), information and education about treatment options, peer support, and coordinated linkage to treatment and/or services. Also offers a Safe Place to Stay providing food, shelter, and professional support while individuals are waiting for appropriate treatment placement in the community.

Provides Early Intervention Service (EIS) coordination for Allegheny County families. Connects children in need with intervention services provided at their home or day care location.

Provides information and referral services to residents needing to access the state's BH/DS system. Also provides Intake and case management services to assess individual needs, assist individuals in accessing appropriate services, assess financial resources available and coordinate service delivery.

Provides legal representation and case management to victims of abuse and neglect.

Provides comprehensive, direct social service support through professional staff with expertise in sight loss. Services include home visitation and case management, information about available services, assessment, referrals, home management assistance, and counseling.

Addresses the educational, vocational, and community living needs of individuals ages 18-25 years with Psychosocial Rehabilitation, Residential Services, and transitional housing.

Children and Youth staff provide assessment and casework services to families if a parent contacts the agency with concerns regarding their child who has committed a specific act or acts of habitual disobedience of the reasonable and lawful commands of his/her parent, guardian or custodian.

Assists families in linking with the mental health system and to coordinate needed services for children/adolescents. Involves child and family centered Inter-agency Service Planning Team meetings where families meet with service providers, CASSP Coordinators, and others to plan for home, school, and community treatment options. For individuals needing a higher level of care, a case manager will assist the family in choosing a Residential Program that will best meet the needs of the child.

Provides assessment for substance use disorder, referrals for treatment as appropriate, and follow-up case management services.

Provides drug and alcohol services to include prevention, intervention, outpatient, intensive outpatient, case management, and recovery support. Also provides gambling outpatient treatment.

Provides HIV testing, counseling, and case management for at risk adolescents and young adults.

Provides mental and behavioral case management services that include supports coordination, information and referral, consultation and education, counseling, therapy, and family driven support services.

Short term case management for children and adolescents to facilitate connection to community resources

Offers supportive services for older adults under 60 seeking long term independent living. Services include daily living services, community integration, Personal Emergency Response Systems, home modifications, transportation, and supports coordination.

Services include advocacy, needs assessment; developing Individual Support Plan (ISP); locating, coordinating, and linking clients with appropriate community resources; and monitoring the implementation of the ISP for quality assurance and modifications if needed.

The goal of Operation REACH (Relief for Emergency Assistance through Community Help) is to assist veterans who are struggling with substance abuse and/ or co-occurring mental health needs get the help that they need. This initiative will focus on the comprehensive needs of the veteran and delivering services through a holistic approach. Operation REACH will strive to provide intervention support services through a targeted case management model. Veterans and their families will benefit from targeted case management through assistance in coordination with basic necessities. Unlike many programs, characterization of discharge will not serve as eligibility criteria for Operation REACH. Offers case management services to help individuals with health insurance and other benefit applications, basic needs supports via referrals, connections to behavioral health services, and health insurance education and care coordination.

Supports individuals with Intellectual Disabilities and Autism residing in Centre County. All programs and services are designed to support individuals in their own home and/or in their community. Funding options, natural supports and other community resources are explored and developed in order to assist individuals and their families achieve an everyday life.

Provides assistance in accessing needed services and resources, as well as support and advocacy for individuals with substance use issues.

Provides public health education, preventive nursing services, emergency preparedness, immunizations, communicable disease surveillance and investigation, HIV testing and counseling, community empowerment, and other nursing services as mandated by PA legislation. Provides adult and child immunizations, HIV/AIDS referrals, and health resources.

Offers recovery case management coordination (for individuals funded by FFDA for treatment), linkage to community-based recovery support, and coordination of community-based recovery-related events/activities.

Provides case management and information and referral services for those who have recently found out they are HIV positive as well as for those who have been living with HIV for years. Provides individuals with the tools needed to live as healthily and independently as possible with HIV. Connects people to a range of services, support, referrals, and mental health care.

Provides case management for families (parent[s] with children) dealing with homelessness. Families live at host congregations for one week at a time and rotate around to different churches in the area that provide shelter, three free meals a day, and often provide evening activities and fellowship for the guests. Each day, families are transported to the Day Center by the FPHCR bus where families can do laundry, shower, and get ready for school or work. Parents who are not working stay at the Day Center and work on job and housing searches. Each family meets weekly with a staff member at FPHCR for case management.

Offers services including respite care, movement monitors, assistance with obtaining seizure response dogs, scholarships, summer camp for kids, training for teachers/school nurses on seizure recognition, first aid training, sleep and movement monitors/assistance, and care coordination. Provides patient education, information and support for adults, young adults, and children with epilepsy and their families, and those working with individuals living with seizures.
