Home Based Parenting Education
Provides services to support children with complex needs and their families by developing individualized plans of care. The key characteristics of the process are that the plan is developed by a family centered team, is individualized based on the strengths and culture of the child and their family, and is driven by client needs rather than available services.
Supports re-entering citizens upon release from Philadelphia Department of Prisons. Offers paid training in horticulture, landscape maintenance, greenhouse operations, carpentry, and masonry. Training includes job readiness, conflict resolution, health education, and occupational therapy. Also offers social support services. Program graduates receive continued case management support and job placement with partnering employers.
Provides long-term financial assistance with permanent housing. Offers case management for housing, job training and education, financial, physical, emotional, social, and spiritual health.

Meets with each resident and conducts a social history and assessment upon admission. When short-term residents are ready to return home, social services staff helps residents determine the best options for a safe discharge. This includes reliance on home health care, durable medical equipment, community services, and support groups.
Offers the following services to eligible elderly or disabled clients: assistance with personal care and hygiene; homemaker services such as light housekeeping, grocery shopping, and meal preparation; transportation; respite services; companion services; and case management. Also offers the Senior Health Specialist Program through which registered nurses provide health screenings and counseling, disease management education, nutrition education, and medication review for senior clients.

Comprehensive Outpatient Substance Use Disorder Treatment
Serves individuals who have had extended hospital stays and/or complex service needs and are at high risk of re-hospitalization by providing less restrictive community-based support including medication management, connection to case management services, residential services and rehabilitation/treatment services. Participants will be supported in the home and/or community morning through evening up to seven days a week.
Provides individual case management and connects clients to medical resources, transportation services, and emergency financial assistance.
Assists clients in identifying, facilitating, and coordinating available services, and educating clients on available resources and how to access them. Provides assistance in all areas of the individual’s life, including housing, physical and mental health care, education, employment, and recreation. Also acts as a liaison between the client’s service providers.

Provides comprehensive, direct social service support through professional staff with expertise in sight loss. Services include home visitation and case management, information about available services, assessment, referrals, home management assistance, and counseling.
Acts as the entry point to mental health services in Washington County. Services include, but are not limited to the following: Acute Inpatient Behavioral Health, Outpatient and Partial Hospitalization Treatment, Mobile Medication, Assertive Community Treatment Teams, Supported Employment, and a variety of other recovery-oriented services such as Psychiatric Rehabilitation, and Clubhouse and Peer services. Multiple residential and housing options are available as well for those who meet the criteria.
