Offers services for individuals with substance use disorders. Provides screening and level of care assessment for substance use disorder, referral to treatment, and support until admission if space is not available in an appropriate setting immediately. Care management includes connections to other human services (through warm hand-offs), information and education about treatment options, peer support, and coordinated linkage to treatment and/or services. Also offers a Safe Place to Stay providing food, shelter, and professional support while individuals are waiting for appropriate treatment placement in the community.

Provides Early Intervention Service (EIS) coordination for Allegheny County families. Connects children in need with intervention services provided at their home or day care location.

Office of Mental Health provides treatment and supports for residents with serious and persistent mental illness.

Providing career services, placement, and reentry services. After enrollment program details, including participant responsibilities, are reviewed as well as any barriers the participant has that need to be addressed such as securing identification (photo ID, driver's license, birth certificate, social security card., etc.). While the participant is still incarcerated (pre-release), job readiness services are provided along with career exploration and discussions of education and occupational skills training opportunities. Post-release we continue to coordinate education, training, and employment activities while also encouraging participants in their continuation with any, court ordered or voluntary, mental health services and/or drug and alcohol treatment. We also continue to provide supportive services such as bus passes, work clothing, etc. From the date of initial enrollment, we are in contact with participants for about two or more years.

Provides case management services for Migrant, Immigrant, Refugee, and Asylee families connecting them to resources in their community

Provides a variety of services to assist individuals with intellectual and/or developmental disabilities and their families to live and work in their home communities. Support services offered are based in the local community and encourage active participation by the individual, their family, friends, and neighbors. Services include: residential and in-home care, vocational and employment services, family support and specialized therapies, and specialized early intervention services for children from birth to age 3 who have a developmental delay.

Offers intensive, client-centered case management approach to empower families experiencing poverty and address root causes preventing them from becoming more self-sufficient. Helps enrolled families connect with community resources, develop personalized action plans, and objectively track their progress over time.

Offers mental health and community supports to individuals suffering from chronic, severe mental illness who are re-entering society after incarceration.

Provides clients with the resources to enhance the process of recovery by lessening the stresses of early recovery and allowing for a deeper and more meaningful aftercare program. Primary services ensure the following emergent needs are cared for: detoxification, prenatal care, perinatal care, or psychiatric service.

Offers a variety of intake and in-home services designed to support children and families within the community.

Offers a primary healthcare center, which is managed by nurse practitioners in collaboration with physicians. Also provides counseling services when appropriate. Our counseling services help deal with: aging parents, finances, grief/loss, serious illness, sudden change in lifestyle, planning for the future and personal concerns, including: stress, anxiety and depression.

Offers case management services to assist HIV-positive individuals with obtaining medical care, insurance, public benefits, food, housing, mental health treatment, substance abuse treatment, and other resources.

Offers juvenile delinquency prevention program with case management. Provides individual or group meetings in the community, home, or school. Meetings are goal-oriented and individualized to the needs of clients.

Provides public health education, preventive nursing services, emergency preparedness, immunizations, communicable disease surveillance and investigation, HIV testing and counseling, community empowerment, and other nursing services as mandated by PA legislation. Provides adult and child immunizations, HIV/AIDS referrals, and health resources.

Provides non-medical, in-home care, or in-facility care for seniors. Services include Alzheimer's & Dementia Care, Parkinson's Care, Companion Care, Personal Care, Transitional Care, Respite Care, and End-of-Life Support Care.

Offers one on one case management. Provides referrals to local agencies/help.

SAM provides case management services to individuals enrolled in the early intervention, mental health and intellectual disabilities case management program.

A supports coordinator is responsible for the development of a comprehensive Individual Support Plan (ISP). This plan is updated annually and/or when an individual's needs change. Provides ongoing oversight to ensure that desired outcomes are met, as well as monitoring services, and support to ensure health and safety.

Acts as the entry point to mental health services in Washington County. Services include, but are not limited to the following: Acute Inpatient Behavioral Health, Outpatient and Partial Hospitalization Treatment, Mobile Medication, Assertive Community Treatment Teams, Supported Employment, and a variety of other recovery-oriented services such as Psychiatric Rehabilitation, and Clubhouse and Peer services. Multiple residential and housing options are available as well for those who meet the criteria.

Provides drug and alcohol services to include prevention, intervention, outpatient, intensive outpatient, case management, and recovery support. Also provides gambling outpatient treatment.

Case management services for adults with behavioral health issues identified as serious mental illness. Case managers develop recovery plans for individual clients, arrange for required services, monitor services and serve as an advocate for the individual. Offers mental health support services including information/education, intervention, self management and transitional services. Also provides post disaster mental health services.

Community Integration is a service for those individuals who will be transitioning back into their community, or to a new community from a nursing facility or similar.
