LIFE (Living Independently For Elders) St. Mary is a Program of All-inclusive Care for the Elderly (PACE). LIFE St. Mary is an innovative program of care for people age 55 and older with chronic health problems and/or memory loss. LIFE assists older adults in navigating the healthcare system and addressing the needs of the whole person. Services include primary and specialty medical care, medications, personal care at home, activities at a day center, meals and transportation. The LIFE Center provides one location where doctors, nurses, and other healthcare professionals offer treatment and monitor changes in health, where participants receive nutritious meals, activities to keep active and maybe even make new friends. Transportation to the LIFE Center is provided. The goal of LIFE St. Mary is to provide services so aging adults can avoid moving to a nursing home and continue to live at home.
