Program that supports the purchase of case management and other home and community based services as a means of helping people avoid premature institutionalization. Funding is usually through state and local resources, but may also include some federal dollars. The programs typically provide case management and, unless other funding sources are available, provide funding for and link program participants to existing home and community based services, home modification programs, assistive technology equipment, assisted living facilities or other supported housing options, and/or other needed services. Individuals must meet certain eligibility criteria as specified by the program and usually must have documentation from a physician stating that they are at risk for institutionalization.

Addresses truancy issues through prevention and intervention services. Collaborates with Fayette and Westmoreland counties with schools, parents, students, and the court systems. Participation is voluntary, and services can stop at any time.

Designed to promote emotional and social competence and to prevent, reduce, and treat aggression and social emotional problems in children ages. By treating these behaviors, it reduces the chance of developing later delinquent behaviors. It consists of a 2 hour session once a week for 11 weeks. In these sessions, trained facilitators use videos to encourage group discussion, sharing of ideas and problem solving.

Provides supportive services to very low-income Veteran families living in or transitioning to permanent housing. May provide eligible Veteran families with outreach, case management, and assistance in obtaining VA and other benefits.

Levels of Care Assessments are completed as requested by the court system as well as those seeking services voluntarily.Treatment in the Drug and Alcohol Program can include various interventions based upon the needs of the participant

Assists older adults who have difficulty with their financial affairs and problems maintaining their finances. Trained volunteers prepare bills for payment, assist with check writing, and balance checkbooks.

Offers specialized camps for adults, youth, and veterans with disabilities, focusing on independence, personal growth, and social skills, during the summer months from June through August.

This program provides free groceries and essential food items to eligible individuals and families in need.

Offers primary health care services for individuals of all ages. Specialty services include pediatrics, OB/Gyn, and podiatry.

Provides assistance to older adults with various health issues or those who need companionship. Supports socialization, helps prepare lunch, play a game, read, or accompany senior to events or appointments. Senior companions are matched with clients according to needs, abilities, and distance.

Municipal office

Provides intensive mental health treatment to individuals through group and individual psychotherapy, psycho-educational groups, psychiatric consultation and medication management. Day Services offers supports for individuals who have been diagnosed with serious mental illnesses, developmental disabilities and have a history of problem sexual behavior to help live a safe and satisfying life. Topics addressed include:

• Anger management
• Sexuality
• Self-esteem
• Relapse prevention
• Drug and alcohol issues

Provides an array of services for adults, including partial hospitalization program (PHP), intensive outpatient program (IOP), and comprehensive psychiatric services. Services provided in-person and via telehealth and online.

Community emergency response team.

Offers a program that runs from June to August and takes over where school-year programs end, providing seven days of breakfasts and lunches in a convenient weekly drive-through format.
