Individuals must meet certain eligibility criteria as specified by the program and usually must have documentation from a physician stating that they are at risk for institutionalization.

Provides end-of-life care and support to terminally ill individuals and their families or caregivers. Services can be provided in a home setting or at an extended care facility. The hospice team includes nurses, social workers, spiritual counselors, therapists, hospice aides, volunteers, physicians, and bereavement counselors.

Provides emergency assistance for such things as food, clothing, shelter, rent, household utility expenses, and similar miscellaneous living needs. May offer seasonal assistance, such as Back-to-School and Project Bundle-Up, when fund are available. Referrals are made for any additional assistance not offered by the agency.
