Provides quality care for the individual with a disability while giving family members short-term relief from their day-to-day care giving routines. On a per-arranged basis, qualified care providers come to the home and care for the person or take them out in the community.

Offers short term service designed to supervise and support individuals living in private homes. Services may be needed due to the absence or need for relief of those persons normally providing care. Respite can be provided either daily or on a 15-minute basis.

Provides non-medical, in-home care, or in-facility care for seniors. Services include Alzheimer's & Dementia Care, Parkinson's Care, Companion Care, Personal Care, Transitional Care, Respite Care, and End-of-Life Support Care.

Offers respite services to those caring for an adult with behavioral or mental health needs by providing temporary accommodations or in-home care for your loved one, with a qualified Access Services host family, capable of meeting all of his or her personal needs.

Provides a Bed and Breakfast where family caregivers and the bereaved can stay overnight and take advantage of nature-focused services to help them manage stress, restore balance, and improve their physical, mental, and emotional health.

Provides family caregivers and their loved ones a break from the typical care routine, while loved ones benefit from care and support provided within a safe, nurturing environment. Respite care services require a minimum five-night stay and may include: 24/7 on-site care provided by nurses and nursing assistants; assistance with personal care; medication management; social services support; all meals and daily housekeeping services, activities and special entertainment.

Provides short term, temporary care for families who need an interval of relief from the daily challenges that come with caring for a child with mental and behavioral health needs.

A financial management/participant-directed service available to individuals with an intellectual disability who reside in their own residence or the home of the family. Services may include: Home and Community Habilitation, Companion, Respite, Camps, Home and Vehicle Adaptation.

Evening Respite offers care and recreation for children and adults with disabilities on Fridays from 6-10pm. We accept all age groups and siblings as well. This program is currently being offered on Friday evenings only, but may be expanded, based on consumer demand. The location of the respite site alternates from week to week.

Staff meet with youth and families multiple times a week at times most needed by the family, with an emphasis on safety and support. Individualized service plans balance involuntary service demands with activities driven by the family's prioritized wants and needs.

Provides lifesharing and respite services in private residential homes.

Provides a palliative approach to hospice care. A team of experienced health care professionals offers much needed help to anyone during this difficult time. Addresses physical, emotional, spiritual, and psycho-social needs which are unique to each individual and family. Works closely with your physician to address your specific goals. If no personal physician, one of the team physicians will direct care.

A 32-bed sub-acute care facility for medically-fragile or technology-dependent individuals who may or may not have a developmental disability. Offers respite care, summer camp, transitional care, and long term care

Provides independent living and assisted living services for older adults. Short and long term care, respite, rehabilitation, post-hospital, and vacation services are available.

Offers out-of-home Respite Care Services when vacancies are available in an appropriate group home.

Offers adult day programs and care. Provides support and temporarily relief for caregivers. Participants can choose to attend programs as often as they would like. Flexible scheduling like half or full days. Programming includes: Exercise classes, Educational presentations, Medication administration, Bathing assistance, Personal care assistance, etc.

Provides short and long term care in various residential sites. Potential respite individuals are placed based on capacity and "fit" for the program. Matches are based on the referral and the current occupants of the home.

Meets client's individual needs and interests and provides a trained staff member to assist clients in home-based habilitation, personal care services, companion services, community integration, and respite services.
