County Correctional Facility

A state and county funded agency mandated by law to provide services for persons with intellectual disabilities. Works closely with many other agencies, bureaus and providers of these services. Manages the State Medicaid Waiver Program. Supports Coordinators assist individuals with intellectual disability to connect with various services:

• Family Support Services: respite care, family aide, nursing care, home and community habilitation
• Residential Services: supervised apartments, semi-independent living, group homes, family living/life sharing
• Vocational Training: Job skills, obtain and maintain a job, workshop setting, transitional employment, volunteer work, job search and placement, supported employment, job loss prevention
• Adult Day Services: Adult developmental training focusing on basic life skills and leisure activities, community habilitation, senior programs

Provides Adult Day Programs that offer supervision and activities geared to the abilities of the participants. Provides therapy, recreation, meals, and support for those seniors who require constant supervision or for those seniors who need socialization opportunities but are too physically or mentally impaired to attend a senior center.

Medical Assistance, also known as Medicaid, pays for health care services for eligible individuals.

Provides case management for families (parent[s] with children) dealing with homelessness. Families live at host congregations for one week at a time and rotate around to different churches in the area that provide shelter, three free meals a day, and often provide evening activities and fellowship for the guests. Each day, families are transported to the Day Center by the FPHCR bus where families can do laundry, shower, and get ready for school or work. Parents who are not working stay at the Day Center and work on job and housing searches. Each family meets weekly with a staff member at FPHCR for case management.

Provides Learning Locomotion, an affiliate child care program, located on the CIT site offering childcare for children, ages 2-5 years, also providing a hands-on training for students learning to provide childcare.
