Provides low-income/ subsidized private rental housing for individuals experiencing chronic mental illness.

Assists with making case corrections (birth date, social security number, name), address & telephone # changes, individual add or deletions (newborns, household member moves in or out, deaths), new employment or termination of employment, income changes, change in household expenses - rent & utilities, request forms and documents, and answer general questions

Provides twenty-four hour personal care supportive services, assisting consumers with their Activities of Daily Living, including assistance with medication administration and bathing and dressing, whether long-term placement or a short-term care, a variety of services are available to enable consumers to achieve their highest practicable level of physical, emotional, and spiritual wellbeing.

Provides light housekeeping, meal preparation, laundry, respite care, and/or personal care for the elderly, new mothers or accident victims.

Provides Certified Peer Specialist services such as mentoring, advocacy and mediation, engagement, wellness strategies, and mental health education.

Nutrition counseling program supplements the care patients receive from their primary care providers. During counseling sessions, a dietitian will work with patients to assess current nutritional habits and create action plans for the future. The goal of nutrition counseling program is to enable patients to take control of their habits and health.

Offers a variety of health-related programs for older adults, including: health awareness classes, brain fitness classes, ask-a-nurse program, free hearing screens, blood pressure checks, and flu shots.

Accepts complaints from citizens about police conduct.
